Optical coherence tomographic image of an angiographically borderline lesion with a significant fractional flow reserve.

چکیده
42-year-old diabetic woman presented with left ventricular dysfunction late after an Aanterior wall myocardial infarction. Fluoro-deoxy glucose-positron emission tomography showed viability in the left anterior descending artery (LAD) territory. Coronary angiogram (A, Online Video 1) showed a significant mid-LAD disease which was stented. Fractional flow reserve (B) done for the borderline lesions in the right coronary artery (RCA) was significant (0.53). Optical coherence tomography (C, D, E) done for the RCA lesion showed lotus root–like appearance (1) with multiple channels of varying diameters corresponding to the hazy lesion in the mid-RCA. The contrast flowing through these channels creates an illusion of a near normal luminogram even on different angiographic projections. Percutaneous coronary intervention to RCA was done successfully with drug-eluting stent (F, Online Video 2). This case best showcases the limitations of luminogram obtained by conventional angiogram and even a focal haziness without narrowing could be significant stenosis.

Fractional flow reserve or optical coherence tomography guidance to revascularize intermediate coronary stenosis using angioplasty (FORZA) trial: study protocol for a randomized controlled trial
BACKGROUND The management of patients with angiographically intermediate coronary lesions is a major clinical issue. Fractional flow reserve provides validated functional insights while optical coherence tomography provides high resolution anatomic imaging. Both techniques may be applied to guide management in case of angiographically intermediate coronary lesions. Moreover, these techniques ma...
متن کاملCritical analysis of the correlation between optical coherence tomography versus intravascular ultrasound and fractional flow reserve in the management of intermediate coronary artery lesion.
The appropriate assessment of intermediate coronary artery stenosis continues to be a challenge for cardiologists. Several studies have shown that anatomic parameters obtained by intravascular ultrasound (IVUS) and optical coherence tomography (OCT) showed a correlation with fractional flow reserve (FFR) values in identifying hemodynamically severe coronary stenoses. However, the efficacy of IV...
متن کاملApplication of fractional flow reserve and optical coherence tomography examinations in a patient presenting with recurrent angina: a case report
INTRODUCTION We present the different roles of fractional flow reserve and optical coherence tomography in guiding treatment in a patient with recurrent chest pain. CASE PRESENTATION A 66-year-old Chinese woman presented to our department for the third time for her recurrent chest pain. Her physical examination was unremarkable; her previous two angiography examinations indicated that there w...
متن کاملRelationship between Quantitative Optical Coherence Tomography Parameter and Pressure-derived Fractional Flow Reserve for Assessing the Functional Severity of Coronary Stenosis
Background: Optical coherence tomography (OCT) allows us to evaluate not only the qualitative coronary lesion morphology but also the quantitative anatomical severity of coronary stenosis. Fractional flow reserve (FFR) is an established index to evaluate the functional severity of a coronary stenosis. The aim of this study was to determine the OCT parameters for physiological severity of corona...
