Quantitative assessment of diffuse myocardial fibrosis in II-type diabetes mellitus patients using T1 mapping technique: preliminary data

چکیده
Background In diabetic cardiomyopathy (DCM), ventricular remodelling consists in a progressive impairment of myocardial contraction (evolving from diastolic to combined diastolicsystolic dysfunction) occurring regardless of ischemic heart disease, hypertension or other macrovascular complications, which ultimately leads to heart failure. Early stages of DCM are asymptomatic and characterised by initial contractile disfunction and various degree of myocardial fibrosis, that may not be recognised by traditional cardiology tests. Our purpose was to detect myocardial fibrotic infiltration in DM-II patients by using T1-mapping technique with extracellular volume fraction (ECV) measurement.

Diffuse myocardial fibrosis and early strain abnormalities in asymptomatic type 2 diabetics without overt cardiovascular disease
Background Type 2 diabetes has been associated with a higher prevalence of diastolic dysfunction and congestive heart failure. A hypothesized contributor is the accelerated accumulation of diffuse myocardial fibrosis. In a cohort of type 2 diabetic patients without overt cardiovascular disease, we sought to explore the capacity of quantitative T1 mapping and feature-tracking based strain analys...
متن کاملIncreased myocardial extracellular volume assessed by cardiovascular magnetic resonance T1 mapping and its determinants in type 2 diabetes mellitus patients with normal myocardial systolic strain
BACKGROUND Cardiac magnetic resonance (CMR) T1 mapping and tissue-tracking strain analysis are useful quantitative techniques that can characterize myocardial tissue and mechanical alterations, respectively, in patients with early diabetic cardiomyopathy. The purpose of this study was to assess the left ventricular myocardial T1 value, extracellular volume fraction (ECV), and systolic strain in...
متن کاملNative T1 mapping in patients with idiopathic dilated cardiomyopathy for the assessment of diffuse myocardial fibrosis: validation against histologic endomyocardial biopsy
Background Late gadolinium enhancement (LGE) MRI provides a significant impact on prognosis in dilated cardiomyopathy (DCM) patients. However, LGE MRI is less suitable for quantifying the degree of fibrosis in diffusely diseased myocardium. T1 mapping technique allows for the quantitative assessment of extracellular volume fraction (ECV), which has been histologically validated against the coll...
متن کاملT1 Mapping for Myocardial Fibrosis by Cardiac Magnetic Resonance Relaxometry—A Comprehensive Technical Review
Cardiac magnetic resonance (CMR) imaging has been widely used to assess myocardial perfusion and scar and is the non-invasive gold standard for identification of focal myocardial fibrosis. However, the late gadolinium enhancement technique is limited in its accuracy for absolute quantification and assessment of diffuse myocardial fibrosis by technical and pathophysiological features. CMR relaxo...
