چکیده
The purpose of this Capstone Design Project is to create an improved system for transporting patients from one hospital unit to another for the Veterans Health Administration (VHA). This new process is a paperless system that uses computerized forms to communicate between departments, electronic messages to assign tasks to employees, and automatically-generated control charts to monitor the process. The goal of this project is a new Escort Service system that will decrease response time, reduce the frequency of miscommunication errors, and allow management to monitor and control the department’s performance. An improved process was designed, a computer database was created, and the new system was implemented and studied at the West Roxbury (MA) Campus of the VA Boston Healthcare System. U.S. Department of Veterans Affairs The Need for Project The VHA is looking to create an improved system for transporting patients from one unit of the hospital to another. There is a critical need for process improvement of healthcare systems in the United States. Ineffective systems for medical care create exorbitant costs and avoidable errors that harm patients. The Veterans Health Administration (VHA) is a branch of the U.S. Department of Veterans Affairs (VA) that provides primary care, specialized care, and related medical and social support services to American veterans. For many VHA facilities, the system for transporting patients from one unit of the hospital to another, known as the Escort Service, is filled with inefficiencies and errors. Insufficient processes result in delayed response times, unfilled requests, miscommunications between departments, and low employee morale. An effective Escort Service will have a positive impact on all VHA employees and patients and has the potential for implementation at every VHA hospital across the country. The Design Project Objectives and Requirements The objective of this project is to design an improved process for patient transportation within the hospital. This process involves Nurses requesting Escorts to transport the patient and a Dispatcher assigning these transports to an Escort. Design Objectives The objective of this project is to design an improved process for requesting patient transports and assigning these transports to employees for the VHA. The three objectives of this new Escort Service system are to (1) decrease the response time, (2) reduce the frequency of miscommunication errors, and (3) allow management to monitor and control the department’s performance.

Incidence of patient safety events and process-related human failures during intra-hospital transportation of patients: retrospective exploration from the institutional incident reporting system
BACKGROUND Intra-hospital transportation (IHT) might compromise patient safety because of different care settings and higher demand on the human operation. Reports regarding the incidence of IHT-related patient safety events and human failures remain limited. OBJECTIVE To perform a retrospective analysis of IHT-related events, human failures and unsafe acts. SETTING A hospital-wide process ...
متن کاملCreating a Culture of Patient Safety through Innovative Hospital Design
When SynergyHealth, St. Joseph’s Hospital of West Bend, Wisconsin, decided to relocate and build an 82-bed acute care facility, we recognized the opportunity to design a hospital that focused on patient safety. Hospital leaders believed if a facility design process was “engineered properly,” it would enhance patient safety and create a patient safe culture; however, we found little information ...
متن کاملDesign of a prostate cancer patient navigation intervention for a Veterans Affairs hospital
BACKGROUND Patient navigation programs have been launched nationwide in an attempt to reduce racial/ethnic and socio-demographic disparities in cancer care, but few have evaluated outcomes in the prostate cancer setting. The National Cancer Institute-funded Chicago Patient Navigation Research Program (C-PNRP) aims to implement and evaluate the efficacy of a patient navigation intervention for p...
متن کاملTransport of referred sick neonates: how far from ideal?
Transportation of sick neonates under controlled conditions has a direct relationship with morbidity and mortality(l). Poor transportation is one of the iatrogenic factors significantly associated with greater neonatal mortality(2). Attention is now being paid to intra-hospital transport of babies. However, care during referral to hospital is largely a neglected field in India and no documentat...

CONTEXT Although home-based health care has grown over the past decade, its effectiveness remains controversial. A prior trial of Veterans Affairs (VA) Team-Managed Home-Based Primary Care (TM/HBPC) found favorable outcomes, but the replicability of the model and generalizability of the findings are unknown. OBJECTIVES To assess the impact of TM/HBPC on functional status, health-related quali...
