چکیده

Although studies suggest that fasting does not prevent the risk of “ab ingestis” respiratory disease, pre-operative oral maltodextrin seems to help reduce insulin resistance and improve post-operative outcomes in patients undergoing gastrointestinal surgery. In our large series of patients treated with a fast-track pathway, patients were permitted to eat solids until 8 hours before surgery and consume clear liquids 2 hours before surgery, with no gastroenteric or respiratory complications, better psychophysical responses, and good, rapid passage of stools. In this study, we aimed to evaluate the effect of the fast-track pathway on post-operative insulin resistance

There are more than 140 million people with diabetes in the world. Iran’s share is estimated at 1.5 million people. The increasing prevalence of diabetes and the longer life expectancy of diabetic patients mean that an increasing number of patients with diabetes are undergoing surgery, and not just for diabetes and its complications, such as end-stage renal disease, retinopathy, peripheral vas...

INTRAPERITONEAL AND INCISIONAL BUPIVACAINE ANALGESIA FOR MAJOR ABDOMINAL/GYNECOLOGIC SURGERY: A PLACEBOCONTROLLED

ABSTRACT Background: Postoperative pain is an important surgical problem. Recent studies in pain pathophysiology have led to the hypothesis that with perioperative administration of analgesics (pre-emptive analgesia) it may be possible to prevent or reduce postoperative pain. This study was planned to investigate the efficacy of pre-emptive analgesia on postoperative pain after major gynecolog...

Intra-operative administration of low-dose IV glucose attenuates post-operative insulin resistance.

BACKGROUND & AIMS Insulin sensitivity often decreases after surgery in spite of normal insulin secretion, and may worsen the outcome. This post-operative insulin resistance increases according to the magnitude of surgical invasion. However, supplementation of carbohydrates before surgery attenuates the post-operative insulin resistance. This study aimed to investigate the effect of intra-operat...

Preoperative oral carbohydrate therapy.

PURPOSE OF REVIEW Management of the postoperative response to surgical stress is an important issue in major surgery. Avoiding preoperative fasting using preoperative oral carbohydrates (POC) has been suggested as a measure to prevent and reduce the extent to which such derangements occur. This review summarizes the current evidence and rationale for this treatment. RECENT FINDINGS A recent r...

Modulation of post-operative insulin resistance by pre-operative carbohydrate loading.

Insulin resistance develops as a response to virtually all types of surgical stress. There is an increasing body of evidence that suggests that insulin resistance in surgical stress is not beneficial for outcome. A recent large study in intensive-care patients showed that aggressive treatment of insulin resistance using intravenous insulin reduced mortality and morbidity substantially. Similarl...
