چکیده

Aim: To elucidate the outcome for very low birth weight infants delivered after preterm premature rupture of membranes (PPROM) managed conservatively, and to determine the prognostic value of perinatal factors for long-term outcome. Methods: Perinatal data were collected from medical records for singleton live-birth infants delivered between 1991 and 2008 after conservatively managed PPROM. Cases of congenital anomalies and chromosomal aberrations were excluded. Poor outcome was defined as a composite measure of death, neurological morbidity including cerebral palsy, or neurodevelopmental delay. Associations between perinatal factors and poor outcome at the corrected age of 3 were estimated using logistic regression analysis. Results: After the exclusion, 356 infants meeting the study criteria were identified, 26 cases were lost to follow-up, and 330 were eligible at 3 years. The mortality rate was 10% and the incidence of neurological morbidities was 12%. Logistic regression analysis revealed gestational age at birth [adjusted odds ratio (aOR) 0.815], and five-minute Apgar scores (aOR 0.521) were independent predictors for poor outcome. Conclusion: When PPROM was managed conservatively, 22% of VLBW infants had a poor outcome at corrected age of 3 years. Early gestational age at birth and low Apgar scores were associated with poor outcome. Corresponding author.

Effects of Progesterone on Latency Period in Patients with Preterm Premature Rupture of Membranes during 24-34 Weeks of Pregnancy

Background & Aims: Preterm premature rupture of membranes (PPROM) is the leading cause of preterm birth and perinatal mortality and morbidity, and complications. The objective of the present study was to assess the effect of progesterone agents (rectal suppositories and weekly injection of 17- hydroxyprogesterone caproate) on latency period in women with PPROM. Methods: This randomized, double-...

Is it possible to make a reliable prognosis within the first hour of life for very low birth weight infants delivered after preterm premature rupture of membranes?

BACKGROUND One third of all preterm births are due to preterm premature rupture of membranes (pPROM). An accurate prognostic evaluation after admission to the neonatal intensive care unit is necessary. OBJECTIVE The aim of this study was to identify prognostic factors within the first hour of life for mortality, short-term pulmonary morbidity, chronic lung disease (CLD) and severe cerebral mo...

An Extremely Premature Neonate with Severe Anemia

Spleen rupture in an extremely premature newborn is very rare event. High index of suspicion is required to make timely diagnosis and thereafter appropriate management. We present a rare case of an extremely premature, extremely low birthweight newborn who presented with severe anemia secondary to splenic rupture. It was managed conservatively without splenectomy resulting in complete resolutio...

Effects of Probiotic Lactobacillus Reuteri (DSM 17938) on the Incidence of Necrotizing Enterocolitis in Very Low Birth Weight Premature Infants

Background: Feeding intolerance is a common problem among premature infants. There is limited information on the safety and effects of oral probiotic supplements, especially products containing Lactobacillus reuteri, and the incidence of necrotizing enterocolitis (NEC) in low birth weight preterm infants. This study aimed to evaluate the effects of Lactobacillus reuteri on the gastrointestinal ...

Frequency of Thyroid Function Disorders among a Population of Very-Low-Birth-Weight Premature Infants

Background: Thyroid function disorders, particularly congenital hypothyroidism (CHT), are important endocrine dysfunctions associated with permanent morbidities. CHT is more prevalent among preterm low-birth-weight neonates compared to term infants with normal weight. Methods: This prospective cohort study was conducted on 126 very-low-birth-weight (VLBW) neonates referred to the neonatal inten...
