Metabolic interrelationship between vitamin B12 and ascorbic acid in pernicious anemia.

چکیده
S UBNORMAL PLASMA ascorbic acid concentration and rapid plasma ascorbate clearance have been found in patients with vitamin B12 deficiency despite adequate intake of vitamin C. 2 Cox et al.2 noted in their group of vitamin B12-deficient patients that these abnormalities of ascorbate metabolism were still present 7-14 days following therapy with sufficient vitamin B12 to cause clinical and hematological response. Prolonged vitamin B12 therapy was necessary before ascorbate metabolism returned to normal.2 Recently we described a patient with scurvy and anemia in whom, during therapy with vitamin C, methylmalonate (MMA) excretion was detected.3 At this time, the hemoglobin concentration was rapidly rising but the plasma vitamin B12 activity was normal. It was speculated that MMA excretion suggested depletion of body vitamin B12 stores, since therapy with vitamin B12 caused abolition of MMA excretion.4’5 In this patient despite long term administration of high doses of vitamin C, the plasma ascorbate concentration did not become normal until MMA excretion was abolished.3 These data suggested that the plasma ascorbate concentration is related to the excretion of MMA and is directly or indirectly related to vitamin B12 stores.

The studies of many investigators have led to a unified concept of the megaloblastic anemias as a single morphologic entity due to defective nucleoprotein synthesis from various causes. The vast majority of patients with megaloblastic anemia have been found to have deficiency of vitamin B12, of folic acid, or of both vitamins. For this reason, the possible interrelations of these two vitamins h...

Subacute combined degeneration ( SCD) due to vitamin B12 deficiency is common in North of Europe but sometimes it is seen in Iran. It can develop before the symptoms of anemia is obvious, with myelin injury and then axon and even neuron destruction in spinal cord, brain, optic and peripheral nerves. In this study two cases are reported; in the first patient, this disease in manifested by vitami...

Background: Vitiligo is an idiopathic and acquired disease characterized by de-pigmented areas of skin. The precise mechanism involved is not clear. Low levels of folic acid and vitamin B12 have been suggested to play a possible role in the pathogenesis of vitiligo. In this study we checked the serum level of folic acid and vitamin B12 in patients with vitiligo. Materials and methods: In this ...

TROPHIC GASTRITIS is a common occurrence in patients with autoimmune thyroid disease.1’2 The severity of the atrophic gastritis varies from achiorhydria with malabsorption of vitamin B12 ( Addisonian pernicious anemia ) ( P.A. ) , through achlorhydria with adequate vitamin B12 absorption, to minimal or normal secretion of gastric acid and intrinsic factor. It was therefore considered that the s...

M EGALOBLASTIC ANEMIA is due chiefly to deficiency of vitamin B12 ( B12 ), of folic acid, or of both. Recent studies3’4’12’22 have been focused on the view that the metabolic defect leading to megaloblastosis may, in some way, be related to nucleic acid metabolism. However, the role of B12 coenzymes in mammalian nucleic acid metabolism has not been elucidated, nor are there exact data on the na...
