Extracorporeal Circuit

چکیده
Within the last two years, four pre-bypass filters of relatively the same design and performance characteristics were introduced into the cardiopulmonary bypass market. Two, the Harvey H-600 and the Delta Medical PB-005t pre-bypass filters were previously studied by this author 1• A determination was made of the type and number of particles released from different oxygenation systems. Additionally, pressure gradients generated across the pre-bypass filters at predetermined flow rates were measured. The particles removed by these filters indicated that particles were released from the extracorporeal circuit and that pre-bypass filtration could be advantageous in eliminating this source of microemboli. The present research project was designed to determine particulate size filtration limitations in a prebypass application and to propose the question of where we should set our pre-bypass filtration limits. The filter investigated in this study is the Gelman 0.45 ~m minicapsule filter*, which is, at this time, not a medical device. The present applications of this filter include clarification of aqueous and biological solutions and determination of coliform concentrations in water. A comparison of the design characteristics between the Gelman 0.45 ~m minicapsule filter and the

Severe Clotting During Extracorporeal Dialysis Procedures
The prevention of extracorporeal clotting during hemodialysis maximizes the effectiveness of treatment by maintaining patency in the dialyzer blood compartment and thereby obviating several important consequences. For example, clotting leads directly to blood loss, as much as 200-300 ml for each episode, depending on the total volume of the extracorporeal circuit. In addition, clotting reduces ...

During the course of extracorporeal membrane oxygenation, patients are at constant risk of exposure to air emboli. Air emboli may enter the circuit during routine lab sampling, medication administration, air entrainment through the venous cannula, or via a circuit disruption. Circuit components have been designed and positioned to minimize the quantity of air that travels through the arterial l...
متن کاملMembrane Oxygenation: A Case Report
Amphotericin B treatment during extracorporeal membrane oxygenation is at times necessary for the treatment of secondary fungemia or, less commonly, to treat a primary fungal infection. The impact of the extracorporeal circuit on the pharmacokinetics of amphotericin B have not been described in vivo. This case report describes the pharmacokinetics of amphotericin B in a patient receiving extrac...

Recent findings Since the 1950s, extracorporeal lung support has experienced continuous advancements in circuit design and safety in acute respiratory distress syndrome, chronic obstructive pulmonary disease exacerbations, as a bridge to transplantation, intraoperative cardiopulmonary support, and for transportation to referral centers. Patients on extracorporeal membrane oxygenation are now ca...

Extracorporeal circulation (ECC) circuits vary in design in order to fulfill the needs and perfusion plans of the surgical team. There are as many variations in these configurations as there are perfusion techniques. This article describes a simple circuit modification that has diverse advantages in its application. The implementation of this modification may improve sterile technique, lower...

Pediatric patients who have preoperative hemodynamic instability or postoperative cardiac decompensation may frequently require the use of extracorporeal membrane oxygenation (ECMO) for stabilization of cardiac and respiratory function. While ECMO can be a therapeutic treatment for the congenital pediatric patient, it does not allow the additional functions of a complete cardiopulmonary bypass ...
