چکیده

The aim of this analysis was to compare the functional outcomes, knee stability, and complications of patients after the arthroscopic double-bundle anterior cruciate ligament (ACL) reconstruction with those of singlebundle ACL reconstruction. An electronic search of the PubMed, EMBASE and Cochrane Library databases was performed. The analyzed outcomes were Lysholm scores, Tegner scores, subjective and objective International Knee Documentation Committee Scores (IKDC), Pivot-shift test, Lachman test, KT-1000/2000 measurement and complications. A total of 26 randomized controlled trials were available for this meta-analysis. The final results suggested that the double-bundle ACL reconstruction showed significant better outcomes in both functional outcomes and stability of knee joint, as assessed by the Lysholm scores, Tegner scores, objective IKDC, Pivot-shift test, Lachman test, and KT-1000/2000 (P<0.05). However, we found no evidence of a difference in subjective IKDC and complication rate. Based on current evidence, double-bundle ACL reconstruction appears to yield better outcomes when compared with single-bundle ACL reconstruction. However, the results should be interpreted with caution and further large-scale, well-designed RCTs on this topic are still needed.

Evidence-Based ACL Reconstruction

There is controversy in the literature regarding a number of topics related to anterior cruciate ligament (ACL)reconstruction. The purpose of this article is to answer the following questions: 1) Bone patellar tendon bone (BPTB) reconstruction or hamstring reconstruction (HR); 2) Double bundle or single bundle; 3) Allograft or authograft; 4) Early or late reconstruction; 5) Rate of return to sp...

بررسی میزان پایداری زانو متعاقب بازسازی لیگامان صلیبی قدامی به‌روش دو باندلی و تک باندلی

Background: Anterior cruciate ligament (ACL) reconstruction is a first choice treatment for ACL-deficient knees, and arthroscopic single-bundle reconstruction has been widely accepted around the world in this regard. Although, such single-bundle reconstructions result in sufficient knee stability in most cases, but some patients have not been satisfied with postsurgical results in both short-te...

Comparison of single-bundle versus double-bundle anterior cruciate ligament reconstruction after a minimum of 3-year follow-up: a meta-analysis of randomized controlled trials.

Both single-bundle (SB) and double-bundle (DB) procedures have been widely used in the treatment of anterior cruciate ligament (ACL) rupture; however, the optimal repair strategy remains considerably controversial. In this meta-analysis of published studies, we compared the results of these two techniques. After systematic review of electronic databases and websites, a total of 8 RCTs reporting...

A systematic review of single-bundle versus double-bundle anterior cruciate ligament reconstruction.

INTRODUCTION Double-bundle (DB) anterior cruciate ligament (ACL) reconstruction is becoming increasingly common. However, no definitive data on the superiority of DB reconstruction have been shown when compared with single-bundle (SB) ACL reconstruction. SOURCES OF DATA We performed a comprehensive search of PubMed, Medline, Cochrane, CINAHL and Embase databases using various combinations of ...

Knee rotational laxity in a randomized comparison of single- versus double-bundle anterior cruciate ligament reconstruction.

BACKGROUND While single-bundle anterior cruciate ligament reconstruction reduces anterior-posterior laxity, studies have demonstrated residual rotational instability. Improved pivot-shift results have been shown with the double-bundle graft; however, no study has compared rotational laxity outcome of these surgical techniques in vivo under quantified, isolated torsional loading. HYPOTHESIS Th...
