Anti-N-Methyl-D-Aspartate (Anti-NMDA) Receptor Encephalitis as the Initial Presentation of Small Cell Carcinoma of the Lung in a Middle-Aged Man

چکیده
This case report describes a previously healthy 55 year-old man with unremarkable family and medical history presenting with Cotard delusion, unintentional weight loss, and psychomotor agitation requiring sedation. Testing revealed CSF leukocytosis and erythrocytosis, leading to a preliminary diagnosis of viral herpes encephalitis. As the patient continued to deteriorate, anti-NMDA receptor antibodies were detected in the CSF, yielding a diagnosis of anti-NMDA receptor encephalitis. Chest CT and left hilar lymph node biopsy confirmed small cell carcinoma of the lung. Anti-NMDA receptor encephalitis is a rare paraneoplastic or autoimmune syndrome that may be the sign of an underlying malignancy, as demonstrated in this extraordinarily rare presentation. Caretakers should have a high degree of suspicion for this syndrome in patients with no neurologic or psychiatric history presenting with acute mental status changes.

Anti-N-Methyl-D-Aspartate-Receptor (NMDA-R) encephalitis is a new autoimmune disorder, often paraneoplastic in nature, presenting with complex neuropsychiatric symptoms. Diagnosed serologically, this disorder is often responsive to immunosuppressant treatment. Anti N-methyl-D-aspartate receptor (NMDR) encephalitis has been recognized as the most frequent autoimmune encephalitis in children after acute demyelinating encephalomyelitis (ADEM).However due to the variable the variable clinical presentation, the paucity of specific finding on standard laboratory and radiological investigation remains under recognized. First discribed in 2005, most commonly...

As one of the paraneoplastic limbic encephalitis, anti-NMDA receptor encephalitis, autoimmune mediated, is disclosed by Josep Dalmau of department of neurology of university of Pennsylvania in 2007, though it had been reported since 1981. Young women (aged 14-44, median 27) are susceptible to anti-NMDA receptor encephalitis usually combined with mature cystic teratoma (approximately 54%), in ad...
