چکیده

248 February 2013 I ntroduction of ultrasound as a monitor of needle placement and disposition of the local anesthetic has had a significant impact on the clinical practice of peripheral nerve blocks. in particular, the ability to visualize the interaction between needle and anatomy in real time has led to an increase in the use and success of peripheral nerve blocks. not surprisingly, ultrasound guidance has inspired the development of a multitude of new block techniques, previously unreliable because of the inability to position a needle tip consistently in a tissue plane or adjacent to sensory nerves. A few examples include transverse adductor canal, transversus abdominis plane, and pectoralis blocks; all enthusiastically discussed at regional anesthesia gatherings and online discussion forums.‡ However, before widespread adoption, scrutinizing the novel approaches in high-quality, randomized, controlled trials is necessary to document a favorable risk–benefit ratio. For this reason, the study involving femoral nerve blocks published in this issue of AnestHesi-

Femoral Nerve Block versus Adductor Canal Block for Analgesia after Total Knee Arthroplasty

Inadequate pain management after total knee arthroplasty (TKA) impedes recovery, increases the risk of postoperative complications, and results in patient dissatisfaction. Although the preemptive use of multimodal measures is currently considered the principle of pain management after TKA, no gold standard pain management protocol has been established. Peripheral nerve blocks have been used as ...

Effectiveness of adding ketamine to ropivacaine infusion via femoral nerve catheter after knee anterior cruciate ligament repair

BACKGROUND Elective knee surgery for repairing anterior cruciate ligament is usually associated with moderate to severe postoperative pain, and, therefore, selecting appropriate analgesia can considerably facilitate pain control and patient's discharge. This study was designed to compare the analgesic effectiveness of administration of ropivacaine or ropivacaine plus ketamine on pain control an...

Altered loading during walking and sit-to-stand is affected by quadriceps weakness after total knee arthroplasty.

PURPOSE Total knee arthroplasty (TKA) successfully reduces pain, but has not achieved comparable improvements in function. We hypothesized that quadriceps strength affects performance by altering loading and movement patterns during functional tasks. METHODS Fourteen subjects with isolated, unilateral TKA were tested three months after surgery. Quadriceps strength was assessed isometrically a...

Improvement of walking ability during postoperative rehabilitation with the hybrid assistive limb after total knee arthroplasty: A randomized controlled study

OBJECTIVE We aimed to compare the efficacies of rehabilitation with the hybrid assistive limb and conventional rehabilitation after total knee arthroplasty. MATERIALS AND METHODS A total of 37 consecutive patients who underwent primary total knee arthroplasty for knee osteoarthritis were enrolled. Seven patients withdrew from the study after randomization, and 30 patients (hybrid assistive li...

Femoral Nerve Block versus Intra-Articular Infiltration: A Preliminary Study of Analgesic Effects and Quadriceps Strength in Patients Undergoing Arthroscopic Anterior Cruciate Ligament Reconstruction.

BACKGROUND Adequate analgesia and early rehabilitation is necessary for arthroscopic anterior cruciate ligament reconstruction (A CLR) surgery. OBJECTIVE To compare analgesia and quadriceps strength after femoral nerve block (FNB) with intra-articular infiltration (IA) using 0.25% bupivacaine for ACLR with patella tendon graft. MATERIAL AND METHOD Forty patients were randomized to receive F...
