چکیده

Despite the high prevalence and severe consequences of calcific aortic valve disease (CAVD), its pathogenesis remains relatively poorly understood. For many years, CAVD was considered a degenerative disease, where calcification was a consequence of normal aging. Although it is now widely accepted that CAVD is an active disease process, the erroneous degenerative label contributed to delayed progress in characterizing this condition. At present, our understanding of the mechanisms governing CAVD lags far behind our knowledge of atherosclerosis, a disease to which CAVD is frequently compared. Ultimately, our incomplete understanding of CAVD pathogenesis has profound consequences: other than surgical valve replacement, there remains no treatment to stop the onset or progression of CAVD.

Targeted therapy to prevent progression of calcific aortic stenosis.

Calcific aortic valve disease is common, affecting 25% of adults over the age of 65 years, with progression to severe valve obstruction resulting in more than 50 000 aortic valve replacements annually in the United States. “Degenerative” calcific aortic valve disease was thought for many years to be a passive accumulation of calcium binding to the aortic surface of the valve leaflet. Now, convi...

Notch Signaling in Congenital and Acquired Aortic Valve Disease

Calcific aortic valve disease represents the predominant pathology of tricuspid (trileaflet) and bicuspid aortic valves in developed countries (Ladich et al., 2011). Accounting for approxi‐ mately half of anatomically isolated aortic stenosis and 25 percent of patients with aortic regurgitation (Roberts, 1970), calcific bicuspid aortic valves requiring surgical intervention present at least two...

Molecular and cellular aspects of calcific aortic valve disease.

Calcific aortic valve disease (CAVD) increasingly afflicts our aging population. One third of our elderly have echocardiographic or radiological evidence of calcific aortic valve sclerosis, an early and subclinical form of CAVD. Age, sex, tobacco use, hypercholesterolemia, hypertension, and type II diabetes mellitus all contribute to the risk of disease that has worldwide distribution. On progr...

The vitamin D receptor genotype predisposes to the development of calcific aortic valve stenosis.

OBJECTIVE To test the hypothesis that vitamin D receptor polymorphism is associated with calcific aortic valve stenosis. DESIGN The distribution of one polymorphism of the vitamin D receptor (BsmI B/b) was examined in 100 consecutive patients with calcific valvar aortic stenosis and compared with a control group of 100 patients (paired match for age, sex, and the presence of coronary artery d...

Calcific aortic stenosis: from bench to the bedside--emerging clinical and cellular concepts.

Calcific aortic stenosis is the third most common cause of aortic valve disease in developed countries. This condition increases in prevalence with advancing age, afflicting 2–3% of the population by the age of 65 years. The aging US population has led to a burgeoning number of valve replacements per year, which in turn costs the USA approximately $1 billion. The natural history, as described b...

Spectrum of Calcific Aortic Valve Disease

Calcific aortic valve disease is a slowly progressive disorder with a disease continuum that ranges from mild valve thickening without obstruction of blood flow, termed aortic sclerosis, to severe calcification with impaired leaflet motion, or aortic stenosis (Figure 1). In the past, this process was thought to be “degenerative” because of time-dependent wear-and-tear of the leaflets with passi...
