Giant T Wave Inversion D

چکیده

The genesis of the T wave of the electrocardiogram is ill understood. It represents ventricular repolarization, and is thus dependent on changes occurring at cell membrane level. It is the most labile component of the QRST complex and is affected by many physiol6gical, neuro-humoral, and physical factors in health, as well as being the most sensitive index of disease. One of the most interesting and least understood deformities is that of massive enlargement of the T wave, in which the T wave is grotesquely blown up, often inverted, and covers an area many times that of the QRS complex. Coronary artery disease has often been implicated, especially when the T waves are deeply inverted (Garcia-Palmieri et al., 1956; Ippolito, Blier, and Fox, 1954; Silverman and Goodman, 1949; Pruitt, Klakeg, and Chapin, 1955; Wood and Wolferth, 1934; Fisch, 1961; Costanzo, de la Pierre, and Pietra, 1962), but coronary disease is probably not basically responsible when the deformity is gross. Ippolito et al. (1954) first drew attention to the occurrence of massive T wave inversion with complete heart block, and this observation has been confirmed by others (Pfeferman, Chansky, and Feher, 1960; Szilagyi and Solomon, 1959; Jedlicka and Panos, 1962; Birke and Strom, 1955; Holzmann, 1960; Zoob and Smith, 1963).

Rare Giant T-Wave Inversions Associated With Myocardial Stunning: Report of 2 Cases

Prominent T-wave inversions are well recognized electrocardiographic signs that can occur in acute myocardial infarction (AMI). However, the giant negative T waves may be associated with myocardial stunning without AMI.This case report describes 2 patients without AMI who developed rare giant T-wave inversions measuring up to 35 mm in depth and QT prolongation after admission to hospital. While...

Giant T-wave inversion in patients with acute coronary insufficiency.

We have observed an electrocardiographic (ECG) pattern of deep T-wave or "giant T wave" inversion in patients with acute coronary insufficiency. We reviewed the ECGs of 936 patients admitted to our coronary care unit during a one-year period. We found the pattern of giant T-wave inversion was present in nine patients (1 percent). We examined the echocardiograms of all of these patients and we a...

Giant Precordial T Wave Inversion in a Patient with Gastroenteritis

Giant precordial T wave inversion (GPTI) on ECG may be the result of several pathologies, including myocardial ischemia, pulmonary edema, pulmonary embolism, subarachnoid hemorrhage, apical hypertrophy, and postpacing. We describe a case of a 75-year-old woman who developed GPTI after an episode of gastroenteritis. To our knowledge, this is the first report of this ECG pattern associated with g...

Global T-wave inversion in a 77-year-old woman.

77-year-old woman came to the emergency department with dyspnea and T-wave inversions that became deeper and more diffuse over the next 15 hours (Figure). T waves were inverted, in most leads symmetrically, in all leads except aVR and V1, and the QT interval (604 ms with a QTc of 643 ms) was markedly prolonged. Walder and Spodick described this electrocardiographic pattern as global T-wave inve...

CMR Assessment of the Left Ventricle Apical Morphology in Subjects with Giant T-wave Inversions and Without Apical Wall Thickness>15mm

Background Unexplainable giant T-wave inversions in the precordial leads with apical wall thickness <15 mm has been reported. These patients cannot be diagnosed to have apical hypertrophic cardiomyopathy (AHCM) according to the current criteria. The objective of this study was to evaluate the apical morphology of subjects with giant T-wave inversion in the absence of apical wall thickness>15 mm...

T-wave inversion in the aVL lead is associated with severity of stenosis in the LAD artery

Background & Aims: The clinical value of T wave inversion (TWI) in the lead aVL in diagnosing coronary artery disease (CAD) remains unclear. Hence, we aim to evaluate the association between TWI in the aVL lead and the severity and location of coronary artery stenosis in elective coronary angiography. Material and Method: Electrocardiograms (ECGs) of 167 consecutive patients undergoing electiv...
