چکیده

Introduction Oversensing of extracardiac myopotentials can lead to pacemaker inhibition and loss of cardiac output. We describe 2 similar cases in which oversensing of myopotentials and loss of pacemaker output in pacemaker-dependent patients was caused by a proprietary algorithm designed to prevent T-wave sensing. Switching off the algorithm resulted in return of normal pacemaker function. Knowledge of this algorithm is essential for patient safety and avoidance of a potentially unnecessary invasive procedure to reposition a lead.

Unusual cause for an increase of the sensing integrity counter in a patient with inappropriate implantable cardioverter-defibrillator therapy.

We describe the case of a patient who presented with multiple implantable cardioverter-defibrillator (ICD) shock discharges 12 months after device implantation. Upon device interrogation, intermittent oversensing of electrical noise and potential ICD lead failure were suggested by a significant increase in the sensing integrity counter (SIC), a cumulative count of very short ventricular sensed ...

Hyperglycemia-Induced T-Wave Oversensing as a Cause of Cardiac Resynchronization Therapy (CRT) Failure

T-wave oversensing occurs when the counter starts giving dual beeps for every cardiac cycle instead of one. This usually happens when the monitoring lead displays a tall T wave, which is also sharp. R wave sensing algorithms of the devices do not sense T wave because the slow rate of the T wave is much less than that of the R wave. But the slow rate of T waves may change with time and also beca...

T Wave Oversensing

T wave oversensing is quite a familiar phenomenon to anyone who has watched cardiac monitors for sufficiently long periods in the coronary care unit. The counter starts giving dual beeps for every cardiac cycle instead of one. If it persists during several cycles, the displayed rate goes up and the monitor sounds an alarm indicating 'tachycardia'. This occurs usually when the monitoring lead di...

Authors Reply to 'Strategies for Overcoming T-Wave Oversensing'

In their letter to the editor Cay et al [1] express some valid concerns regarding the strategy we described for overcoming T-wave oversensing in our patient [2]. They stress that programmable features of the device should be used first before planning a reintervention. As mentioned in our case-report we tried both the enhanced T wave suppression algorithm available in the device and programming...

Device rounds: T-wave oversensing: A cause of loss of cardiac resynchronization therapy

A CRT-D patient presented with loss of biventricular pacing associated with heart failure symptoms. The electrocardiogram showed sinus rhythm with alternating wide unpaced and narrower paced QRS complexes. Device interrogation showed T-wave oversensing on all paced biventricular beats, with the following sinus P-wave not tracked due to it falling in the post-ventricular atrial refractory period...
