چکیده

The nutrition support clinician must understand and evaluate all aspects of parenteral nutrition (PN) to maximize benefits while avoiding unfavorable consequences for the patients in the intensive care unit. The various aspects in caring for the PN patient include appropriate patient selection, intravenous access choice and maintenance, and individualized PN prescription to meet each patient's unique macronutrient and micronutrient requirements. Once the PN prescription has been determined, the clinician should also be familiar with the compounding process to ensure the final solution is safe for infusion. PN is one of the most complex solutions prepared in the pharmacy with numerous ingredients. Due to the number of components in the PN solution, physicochemical incompatibilities are a common and serious problem, thus specific compounding sequence and guidelines must be followed to avoid precipitation of mixed products. Once the PN solution is ready for infusion, protocols should be in place to assure safe administration. Close monitoring for tolerance of electrolytes and macronutrients, as well as glucose control is necessary initially; whereas monitoring of specific micronutrients is crucial in long-term PN usage. A standardized process for ordering, preparing, administering and monitoring PN is recommended to assure patient safety.

A Survey on Total Parenteral Nutrition (TPN) in the Intensive Care Unit (ICU) of a Teaching Hospital in Iran

Total parenteral nutrition (TPN) has a vital role in controlling nutritional deficiencies especially in patients with critical conditions who are confined to bed in Intensive Care Unit (ICU). Complete evaluation of the nutritional status of these patients is necessary to achieve their nutritional goal. The aim of this investigation was to study the Complete Blood Count (CBC), coagulation parame...

Early versus late parenteral nutrition in critically ill children.

BACKGROUND Recent trials have questioned the benefit of early parenteral nutrition in adults. The effect of early parenteral nutrition on clinical outcomes in critically ill children is unclear. METHODS We conducted a multicenter, randomized, controlled trial involving 1440 critically ill children to investigate whether withholding parenteral nutrition for 1 week (i.e., providing late parente...

Hyperglycemia and antibody titres against heat shock protein 27 in traumatic brain injury patients on parenteral nutrition

Objective(s):Hyperglycemia worsens the neuronal death induced by cerebral ischemia. Previous studies demonstrated that diabetic hyperglycemia suppressed the expression of heat shock protein 70 and 60 (HSP70 and 60) in the liver. IgG antibody titres against heat shock protein 27 (anti HSP27) were measured to determine whether hyperglycemia exacerbates ischemic brain damage by suppressing the exp...

Timing of (supplemental) parenteral nutrition in critically ill patients: a systematic review

Supplemental parenteral nutrition (SPN) is used in a step-up approach when full enteral support is contraindicated or fails to reach caloric targets. Recent nutrition guidelines present divergent advices regarding timing of SPN in critically ill patients ranging from early SPN (<48 h after admission; EPN) to postponing initiation of SPN until day 8 after Intensive Care Unit (ICU) admission (LPN...

Orally fed patients are at high risk of calorie and protein deficit in the ICU.

PURPOSE OF REVIEW Malnutrition can lead to serious complications in the ICU. Less than half of patients admitted to ICU require ventilation and for many, their optimal route of feeding is oral medical nutrition therapy, rather than enteral or parenteral nutrition. Inadequate oral intake is a prevalent and often difficult problem within this population, as increased calorie deficits are common i...
