Background and Purpose: The management of an infected total knee implant is a major challenge to the orthopedic surgeon. We report the outcome using two-stage reimplantation without cement spacers between stages. Methods: Seven infected total knee replacements (TKR) were treated with the two-stage reimplantation technique from September 1994 through January 2001. Treatment of these patients consisted of thorough debridement and removal of the implants. No antibiotic-impregnated cement blocks or beads were inserted and only a long leg cylinder cast was applied for immobilization between stages. Intravenous antibiotics were used and C-reactive protein (CRP) values were monitored. All of the patients were free of infection and revision TKR was performed without using antibiotic-impregnated cement for fixation at an average interval of 6.3 weeks (range 6–10 weeks). Results: The average length of follow-up was 3.5 years (range 1–7 years) and the average HSS (Hospital for Special Surgery) knee score was 86 points (range 78–94). A good result was obtained in one patient and excellent results in six patients. Average range of motion was 91° (range 80°–100°). No recurrence of infection was noted. Conclusions: Two-stage reimplantation without using a cement spacer between stages for septic total knee replacement is tolerable for patients, simpler for surgeons and can achieve good results without recurrence of infection.

Retaining well-fixed cementless stem in the treatment of infected hip arthroplasty

BACKGROUND AND PURPOSE Two-stage reconstruction, reimplantation after removal of an infected prosthesis, has been considered to be the gold standard for treatment of infected hip arthroplasty. However, during the removal of a well-fixed femoral stem, the proximal femur can be damaged and a sequestrum can be formed, which might lead to chronic osteomyelitis and difficulty in reimplantation. We w...

Two-stage revision of infected total knee arthroplasty using an antibiotic-impregnated static cement-spacer.

BACKGROUND Static antibiotic-impregnated cement spacers have been used widely to treat chronically infected total knee arthroplasties with a high success result. However, difficulty in surgical exposure during revision has been encountered. The purpose of this study was to evaluate the clinical results of two-stage reimplantation of an infected total knee arthroplasty using a static antibiotic-...

Two Stage Hip Revision in Periprosthetic Infection: Results of 41 Cases

BACKGROUND two-stage revision is considered the best treatment approach for the eradication of chronic joint infection. We report the outcome of 41 consecutive patients with infected hip prostheses, treated between 2000 and 2005, with two-stage revision using an antibiotic-loaded cement spacer. METHODS Patients underwent a treatment protocol which included clinical and radiographic evaluation...

Technical Tip for Prosthesis of Antibiotic-Loaded Acrylic Cement (PROSTALAC) Use for Infection after Shoulder Arthroplasty

©2015 by The Orthopaedic Journal at Harvard Medical School BACKGROUND Shoulder arthroplasty procedures have generally excellent clinical outcomes, but infection following shoulder arthroplasty continues to be a difficult problem both diagnostically and therapeutically. Two-stage reimplantation with use of a temporary Prosthesis of Antibiotic-Loaded Acrylic Cement (PROSTALAC) (DePuy, Warsaw, Ind...

Diagnosis Of Persistent Infection In Prosthetic Two-Stage Exchange: PCR analysis of Sonication fluid From Bone Cement Spacers

Introduction: When treating periprosthetic joint infections with a two-stage procedure, antibiotic-impregnated spacers are used in the interval between removal of prosthesis and reimplantation. According to our experience, cultures of sonicated spacers are most often negative. The objective of our study was to investigate whether PCR analysis would improve the detection of bacteria in the space...
