Cytomegalovirus and Development of Cardiac Allograft Vasculopathy: Evidences and Therapeutic Implications

چکیده
Cardiac allograft vasculopathy remains the major cause of long-term failure of heart transplantation. Cytomegalovirus infection was identified as a major risk factor for cardiac allograft vasculopathy development in pioneering studies, even though the possibility that the virus is only an innocent bystander was not completely excluded. Only recently, convincing clinical and experimental evidences support the hypothesis of a direct involvement of cytomegalovirus in cardiac allograft vasculopathy pathogenesis. In this article, we review the mechanisms and clinical evidences supporting the hypothesis that subclinical cytomegalovirus infection leads to adverse long-term graft outcome by favoring cardiac allograft vasculopathy development. In addition, we discuss data pointing to the need for antiviral approaches designed to suppress subclinical cytomegalovirus activation as a long-term strategy to prevent cardiac allograft vasculopathy and chronic allograft damage. Mechanisms of chronic cardiac allograft rejection.
Chronic rejection in the form of cardiac allograft vasculopathy is one of the major factors that affects long-term graft and patient survival after heart transplantation. Whereas multiple factors contribute to the development of cardiac allograft vasculopathy, immunologic mechanisms play the predominant role in the chronic rejection process, because both alloimmune and autoimmune responses are ...

Despite advances in prevention and treatment of heart transplant rejection, development of cardiac allograft vasculopathy (CAV) remains the leading factor limiting long-term survival of the graft. Cardiac allograft vasculopathy etiopathogenesis is not fully understood, but a significant role is attributed to endothelial cell damage, caused by immunological and non-immunological mechanisms. Immu...

OBJECTIVE To study the influence of immune and nonimmune risk factors on the development of allograft vasculopathy after cardiac transplantation. METHODS We studied 39 patients with a mean age of 46+/-12 years. The following variables were analyzed: weight (kg), body mass index (kg/m2), donor's age and sex, rejection episodes in the first and second years after transplantation, systolic and d...

Cardiac Allograft Vasculopathy (CAV) is a serious complication after heart transplantation in adults as well as children and once developed irreversibly compromises the outcome of the recipients. Human leukocyte antigen mismatches, number and duration of rejection episodes, type of immunosuppression and presence of antibody -mediated rejection are among the most relevant immunological risk fact...

BACKGROUND Cardiac allograft vasculopathy is the major limiting factor to the long-term success of pediatric heart transplantation. Cytomegalovirus (CMV) has been shown to be a significant risk factor for the development of cardiac allograft vasculopathy. Recent work has demonstrated CMV DNA in leukocytes in the absence of direct allograft infection, suggesting that vascular changes may not be ...
