چکیده
Aims: To determine whether Percussion, diuresis and inversion (PDI) maneuver following shock wave lithotripsy (SWL) can enhance stone clearance from lower calyx as compared to SWL alone. Methods: 60 Patients with lower pole stones (8-15 mm) were randomized to SWL plus PDI (group A) or SWL alone (group B). Shock waves were delivered to all patients according to a standard protocol. Group A patients underwent PDI maneuver immediately after SWL session. Stone clearance was determined by digital KUB x-ray 2 weeks after SWL. Results: Stone clearance rate (SCR) was 65.5% in SWL plus PDI patients as compared to 34.5% in SWL only Patients. This difference is statistically highly significant (p<.02). Relative Risk (RR) of stone clearance with PDI is 1.9 (95% CI1..07-3.35) as compared to SWL alone. Conclusion: PDI in conjunction with SWL significantly improves stone clearance (65.5%) from lower pole of kidney. This means 1.9 times improvement in SCR with virtually no additional cost.

Efficacy and cost-effectiveness of extracorporeal shock wave lithotripsy for solitary lower pole renal calculi.
PURPOSE We determined the efficacy of extracorporeal shock wave lithotripsy monotherapy and compared its cost-effectiveness with percutaneous nephrolithotomy for the management of lower pole renal calculi. MATERIALS AND METHODS The efficacy (stone-free rates at 3-months posttreatment) of shock wave lithotripsy with the modified Dornier HM3* machine was determined retrospectively in 114 patien...

Background: One of the lithotripsy complications is urinary tract infection (UTI) and sepsis after extracorporeal shock wave lithotripsy (ESWL). The aim was to study the prophylactic effect of antibiotics on UTI after ESWL. Methods: This randomized double-blind clinical trial was carried out on 600 patients admitted to Babol Clinic hospital in 2014-2015. Patients were randomly divided into tre...
متن کاملMannitol-induced diuretic renal ultrasonography: a new technique.
We herein describe a new technique to improve the ultrasonic imaging of nonobstructed stone-bearing kidneys. Mannitol-induced diuresis opens up the collapsed renal collecting system and yields better ultrasonographic imaging by increasing the amount of fluid surrounding the calculi. The potential usages of this technique include ultrasound localization for percutaneous stone extraction and duri...
متن کاملStone clearance in lower pole nephrolithiasis after extra corporeal shock wave lithotripsy – the controversy continues
BACKGROUND To determine factors influencing the clearance of fragments after extra-corporeal shock wave lithotripsy (ESWL) for lower pole calyceal (LPC) stones. METHODS In the period between July 1998 and Oct 2001, 100 patients with isolated lower polar calyceal calculi or= 14 years, were included in the study. Intravenous urograms (IVU) were reviewed to define t...
متن کاملRetrograde intrarenal surgery for renal stones smaller than 2 cm.
INTRODUCTION Extracorporeal shock wave lithotripsy (ESWL) is accepted as the gold standard treatment for renal stones that are smaller than 2 cm. Recently, retrograde intrarenal surgery (RIRS) has been introduced as another form of treatment. We report our experience in dealing with renal stones smaller than 2 cm using RIRS as the primary treatment and following failed ESWL. METHODS A retrosp...
