A tuberculoid-like reaction in lepromatous leprosy; a reactional reversal phenomenon.

-R. CHAUSSINAND

T regulatory cells (TREG)(TCD4+CD25+FOXP3+) distribution in the different clinical forms of leprosy and reactional states*
BACKGROUND Leprosy is characterized histologically by a spectrum of different granulomatous skin lesions, reflecting patients' immune responses to Mycobacterium leprae. Although CD4+CD25+ FoxP3+ T regulatory cells are pivotal in the immuneregulation, presence, frequency, and distribution of Tregs in leprosy, its reactional states have been investigated in few studies. OBJECTIVES This study ai...

BACKGROUND Leprosy is characterized by a spectrum of clinical manifestations that depend on the type of immune response against the pathogen. Patients may undergo immunological changes known as "reactional states" (reversal reaction and erythema nodosum leprosum) that result in major clinical deterioration. The goal of the present study was to assess the effect of Toll-like receptor 2 (TLR2) po...

BACKGROUND Leprosy is a chronic infectious disease caused by Mycobacterium leprae which is an obligate intracellular pathogen. It is characterised by a broad spectrum of clinical forms dictated by the patient's immune response to the organism. The tuberculoid pole has good cell mediated immunity to M. leprae, with few lesions and bacilli while the lepromatous pole has poor immunity coupled with...

A 24 year old man admitted to hospital with borderline lepromatous leprosy was treated with rifampicin, dapsone, and clofazimine. After four months he developed a reversal reaction and the diagnosis was modified to borderline tuberculoid leprosy. The dose of clofazimine was raised and prednisolone added to the regimen without any symptomatic response. His condition improved dramatically after f...

The lepra cell, which Virchow first described and which bears his name, is the anatomic characteristic of the granuloma of lepromatous leprosy. Unna demonstrated that the vacuoles of the foamy cells of Virchow are due to fat in the cytoplasm, and not to edema as was thought by the author who saw them first. Mitsuda (4), in a paper published in 1936, claims to have stained that substance with su...

According to the 1953 Madrid classification (") leprosy presents two polar types, tuberculoid and lepromatous, and two groups, indeterminate and borderline. The chronic evolution of all clinical forms of leprosy, with the exception of the indeterminate form, may be interrupted by acute episodes and sometimes these may constitute the only manifestation of the disease. When these acute phenomena ...
