چکیده

220 July 2016 P mediana dorsalis (PMD) has been rarely visualized but is occasionally implicated as a cause of unilateral epidural local anesthesia. Herein, we demonstrate PMD using epidurography. a normal anteroposterior epidurogram should show contrast outlining the lateral margins of the epidural space bilaterally.1 Herein, we show a unilateral epidurogram through a midline epidural catheter (fig. a) and a contralateral epidurogram through a second epidural needle inserted by contralateral paramedian approach under fluoroscopy (fig. B). although computed tomography and necroscopy have demonstrated some degree of PMD in almost all patients,2 this is rarely a completely intact membrane. although generally asymptomatic, the incidence of symptoms is unclear based on the published data. in figure a and B, there is no spillover of contrast media (solid arrows) across the midline, characteristic of completely intact PMD.3 Unilateral epidural analgesia typically presents as inadequate analgesia with unilateral sympatholysis (warm, dry foot ipsilaterally; cold, clammy foot contralaterally). Most cases of unilateral anesthesia are successfully treated by an additional epidural dose after turning the patient and catheter withdrawal to a depth of 3 to 5cm in the space. Occasionally patients present with persistent unilateral anesthesia despite these measures, where additional epidural dose further extends the unilateral block, causing ipsilateral chest wall and arm anesthesia, with ipsilateral Horner syndrome (see figure, Supplemental Digital content 1, http://links.lww. com/alN/B244). There is often a history of unilateral anesthesia or Horner syndrome in previous labor epidurals.3 Our current practice is to assume that these patients have a completely intact PMD. accordingly, after allowing the initial block to wear off, we empirically perform contralateral paramedian epidural catheterization and manage labor analgesia with a bilateral double epidural catheter technique using two patient-controlled analgesia pumps, each using half the standard dose. This figure highlights that PMD is a potential cause of unilateral epidural analgesia and that a bilateral double epidural catheter technique may be useful in these cases.

Appearance of plica mediana dorsalis during epidurography.

To the Editor: The plica mediana dorsalis (PMD) is thought to be a band of connective tissue, which divides the epidural space at the dorsal midline. Existence of the PMD has been controversial since its first description in 1963 (1). The PMD has been regarded as a potential cause for unilateral epidural blockade (2, 3). For purpose of discussion, we present a case of a patient who presented fo...

Case report: bilateral tunneled epidural catheters to prevent unilateral analgesia for cancer-related pain

OBJECTIVE Unilateral analgesia often occurs with epidural analgesia. Traditional methods of troubleshooting this problem can be insufficient in obtaining adequate pain relief in a timely manner for terminal cancer patients. This case report demonstrates a safe and effective solution which can be utilized in these circumstances. CASE REPORT A 55-year-old female with stage IV pancreatic cancer ...

به دام افتادن کاتتر اپیدورال به هنگام خارج‌سازی: گزارش موردی

Background: Epidural catheters are seldom difficult to remove from patients. The breakage of the catheters is uncommon, troublesome and occasionally dangerous. Case presentation: A lumbar epidural catheter inserted in a 17 year-old man for applying anesthesia for internal fixation of femur fracture and subsequent postoperativeepidural analgesia. In the third postoperative day, duri...

بررسی اثرات مادری و نوزادی افزودن ترامادول به لیدوکایین 2% در بیحسی اپیدورال در سزارین

Background & Aim: Epidural anesthesia is one of the anesthethic techniques for cesarean section. However, epidural anesthesia with lidocaine has some disadvantages such as delayed onset of action and short duration of analgesia. The goal of this study is to evaluate maternal sensory and motor blockade, neonatal Apgar score, and side effects of adding 50 and 100 mg tramadol to 2% lidocaine i...

بی‌حسی کامل نخاعی در بیمار مبتلا به آکندروپلازی: گزارش موردی

Background: Total spinal anesthesia is a complication of lumbar epidural anesthesia following undiagnosed subarachnoid or subdural injection of local anesthetic. Although many achondroplastic dwarfs have a normal spine, catheter insertion may be more problematic with a narrow epidural space making a subarachnoid tap more probable.  Other malformations associated with achondroplasia, such as pro...
