PEG Feeding Tubes - Indications and Management

چکیده
Failure of feeding: Dementia; however, there is insufficient evidence to suggest that enteral tube feeding is beneficial in patients with advanced dementia [6] . There is presently no evidence to suggest long-term survival rates improve in patients with advanced dementia who undergo PEG placement for dysphagia [7] . Cystic fibrosis: PEG feeding is safe, efficacious and acceptable in children and adults with nutritional failure due to cystic fibrosis but should be carried out only in the context of close co-operation between cystic fibrosis chest physicians and an enteral feeding team. Peritoneal dialysis: PEG insertion can improve nutritional status but increases the risk of fungal peritonitis and failure of dialysis. PEG insertion can be undertaken in patients on peritoneal dialysis. Dialysis should be stopped for three days and prophylactic antifungal therapy given. Oro-pharyngeal and oesophageal malignancy: enteral tube placement into the stomach may hinder surgical techniques in oesophageal cancer and should be avoided if curative resection is planned.

Cite this as: BMJ 2010;340:c2414 doi: 10.1136/bmj.c2414 Percutaneous endoscopic gastrostomy (PEG) feeding, introduced into clinical practice in 1980, is now established as an effective way of providing enteral feeding to patients who have functionally normal gastrointestinal tracts but who cannot meet their nutritional needs because of inadequate oral intake. It is the preferred method of feedi...
متن کاملPercutaneous endoscopic transgastric jejunostomy (PEG-J): a retrospective analysis on its utility in maintaining enteral nutrition after unsuccessful gastric feeding
BACKGROUND/AIMS Although percutaneous endoscopic gastrostomy (PEG) is the method of choice for long-term enteral nutrition, feeding-related adverse events such as aspiration pneumonia and peristomal leakage can impede the use of PEG. Percutaneous endoscopic transgastric jejunostomy (PEG-J) using large-bore jejunal tubes may help by circumventing gastric passage during enteral nutrition and impr...
متن کاملEnteral Feeding via Percutaneous Endoscopic Gastrojejunostomy(PEGJ) Tubes Decreases Risk of Aspiration and Tube Dislodgement Related Complications Compared to PEGs
Background: Protein-calorie malnutrition is a common problem that develops in patients requiring management in the surgical intensive care unit (SICU). The use of durable enteral feeding tubes to help meet their nutritional needs has grown dramatically over that past three decades. However, prepyloric enteral feeding has been associated with increased aspiration risks. Hypothesis: We hypothesiz...
متن کاملThe Role of Percutaneous Endoscopic Transgastric Jejunostomy in the Management of Enteral Tube Feeding
Feeding-related adverse events after percutaneous endoscopic gastrostomy (PEG) such as aspiration pneumonia can impede the use of PEG. Percutaneous endoscopic transgastric jejunostomy (PEG-J) using large-bore jejunal tubes with gastric decompression function may improve outcomes by circumventing gastric passage during enteral nutrition and improving drainage of excessive gastric secretions. Thi...
متن کاملPercutaneous endoscopic gastrostomy: An update on its indications, management, complications, and care.
BACKGROUND Numerous disorders impairing or diminishing a patient's ability to swallow may benefit from a PEG tube placement. This is considered the elective feeding technique if a functional digestive system is present. METHODS A PubMed-based search restricted to the English literature from the last 20 years was conducted. References in the results were also reviewed to identify potential sou...
