چکیده

The advent of technology for the delivery of radiofrequency energy via electrode catheters, in the late 1980s, led to the perfection of techniques for the ablation of accessory pathways and classical flutter [1e4]. This has been so successful that these substrates for arrhythmia are rarely seen in the Western world. Two major arrhythmias, however, still remain to be conquereddatrial fibrillation and ventricular tachycardia/fibrillation. The occurrence of atrial fibrillation (AF) seems to be dependent on a number of factors [5]. In most situations there is some form of underlying atrial substrate, which is usually the presence of intraatrial conduction delay. In addition, initiating factors for AF are required and the commonest of these appear to be atrial ectopic beats [6]. The major sources of these ectopic beats appear to be the pulmonary veins [7], although extrapulmonary vein sources are being increasingly reported [8,9]. Numerous recent studies suggest that AF may be treated by the techniques based on ablation within

Cryoballoon Ablation for Pulmonary Vein Isolation of Atrial Fibrillation: A Better Way to Complete the Circle?

Cryoballoon ablation has emerged as a novel tool to perform pulmonary vein isolation in patients with atrial fibrillation. This balloon-based technology has the potential to simplify the procedure by creating continuous circumferential lesion sets with a limited number of energy applications. At the same time, electrophysiologists will encounter new challenges and pitfalls when performing cryob...

Vein of Galen Aneurysmal Malformation and High-output Cardiac Failure in a Newborn

Background: Vein of Galen aneurysm (VGA) is the most common form of symptomatic cerebrovascular malformation in neonates. It develops in a fetus in the first trimester of pregnancy due to unknown reasons, but it is likely to have a genetic etiology. The prognosis of VGA is usually poor, particularly in newborns with heart failure due to high-flow intracerebral shunt. Surgery and endovascular em...

Transient symptomatic ST-segment elevation following transseptal puncture for pulmonary vein isolation

Pulmonary vein isolation (PVI) become a standard procedure in atrial fibrillation (AF) treatment, proved to be more effective than antiarrhythmic medication alone. The procedure of PVI requires access to the left atrium, provided by the transseptal puncture (TSP), which carries potential risk of complication. We describe a case of transient, symptomatic ST-segment elevation in 57 year old male ...

Multi-Electrode Ablation Catheters for AF Ablation: Effective Reality or Elegant Experiment?

The landmark publication by Haissaguerre et al. in 1998 on ablation of PV foci to cure AF, has led the way for a global rise of pulmonary vein isolation for ablation for AF.1 Nowadays in many centres this comprises half of all ablations performed. In 2007, a consensus document was published by Calkins et al.2 on behalf of HRS and EHRA to provide guidelines on how to perform procedures and how t...

Long-term results after the box pulmonary vein isolation procedure for chronic atrial fibrillation in mitral valve surgery.

BACKGROUND We hypothesized that chronic atrial fibrillation (AF) originated from the pulmonary veins, and was maintained by irregular activations of the posterior left atrium. We had performed the box pulmonary vein isolation procedure for the elimination of chronic AF associated with solitary mitral valve disease from 1999 to 2004. This paper evaluated the long-term results of this procedure o...
