چکیده

HAS THERE BEEN PROGRESS IN THE PHYSIOPATHOLOGY OF DISC HERNIATION ASSOCIATED RADICULOPATHY? Since 1934, when a link was demonstrated between disc herniation and sciatica, it has been accepted that compression of the nerve root by disc herniation explained the sciatica. Surgical treatment therefore became standard when medical treatment failed. It now seems that ‘‘chemical’’ factors have a central role in sciatica. The clinical arguments supporting the ‘‘chemical’’ theory are numerous. Laminectomy is sometimes ineffective, the long term success rate being 40– 80%, and reintervention rates are reported to be 5–25%. A considerable amount of asymptomatic disc herniation and severe sciatica without visible root compression has been reported; there is poor correlation between the severity of symptoms and the extent of the disc herniation; and the outcome, frequently favourable, is similar after conservative and surgical treatment. 11

People with lumbar disc herniation and associated radiculopathy benefit more from microdiscectomy than advice in the short term, although there is no difference in the long term.

Lumbar intervertebral disc herniation with radiculopathy (LDHR) is one of the most common clinical entities seen by spine practitioners. Good clinical outcomes have been reported for both surgical and non-operative treatment of lumbar disc herniation with radiculopathy. In the absence of cauda equina syndrome, initial treatment is nonoperative, with approximately 50–60% of patients reporting sa...

Lumbar disc herniation with contralateral radiculopathy: do we neglect the epidural fat?

Lumbar disc herniation (LDH) is the most common cause of radiculopathy, whose pathological entity underlying nerve root compression is usually on the same side as the symptoms. However, LDH causing contralateral radiculopathy are sometimes encountered by pain physicians. There have been tremendous developments in the treatment options for LDH; the situation of LDH causing contralateral radiculo...

Lower thoracic disc herniation mimicking lower lumbar disk disease: A case report

Thoracic disk herniation comprises 0.15% of all disk herniation and has various and confusing manifestations.  Among them, radicular pain down the leg could be the rarest presentation, especially if it is the only complaint. On the other hand, finding the relationship between clinical and paraclinical needs require high index of suspension and it is demanding. A 34-year-old patient, who ha...

Serum Levels of the Inflammatory Cytokines in Patients with Lumbar Radicular Pain Due to Disc Herniation

STUDY DESIGN Cohort study. PURPOSE This study primarily aimed to evaluate the serum levels of tumor necrosis factor-alpha (TNF-α) and interleukin (IL)-4 in patients with lumbar radiculopathy 1 and 12 months after microdiscectomy. OVERVIEW OF LITERATURE Lumbar radiculopathy is possibly caused by inflammatory changes in the nerve root. The intraneural application of pro-inflammatory cytokines...

Low back pain associated with lumbar disc herniation: role of moderately degenerative disc and annulus fibrous tears.

Lumbar disc herniation is one of the most common spinal degenerative disorders which may lead to low back pain (LBP) and radicular leg pain. However, it remains difficult to diagnose a degenerative herniated disc as the LBP generator in clinical practice. The purpose of this study is to explore the characteristic changes of a herniated disc causing LBP on MRI and to clarify the underlying role ...
