Stereotactic Radiosurgery for Treatment of Drug-Resistant Epilepsy: State of the Art and Emerging Applications
نویسندگان
چکیده
Radiosurgery is an emerging and interesting treatment option for several types of drug-resistant epilepsies. Particularly, CyberKnife radiosurgery may represent a therapeutic approach for mesial temporal lobe epilepsies, as well as epilepsies due to hypothalamic hamartomas and arteriovenous malformations. There are many reports in the literature, but precise patient selection, dosimetry, target volume, and expected shortand long-term outcomes are still under investigation. We provide an overview of current and emerging radiosurgical technology, to discuss the biological mechanisms and the clinical applications, and general results of radiosurgery for the treatment of drug-resistant epilepsies. Despite encouraging preliminary results, definitive evidence to support the role of radiosurgery in the treatment of drug-resistant epilepsy is missing and more prospective trials are needed to identify predictors for treatment success in the different types of medically intractable epilepsy. Development of A Head Docking Device for Linac-based Stereotactic Radiosurgery System to be Installed on Neptun 10 PC Linac
Introduction: Stereotactic radiosurgery is a technique for the treatment of intracranial lesions with highly collimated x-ray beam. Linac-based radiosurgery is currently performed by two techniques: couch or pedestal mounted. In the first technique, a device is required to affix patient's head to a couch and moreover to translate it accurately. Structure of suc...
متن کاملLow-dose stereotactic radiosurgery is inadequate for medically intractable mesial temporal lobe epilepsy: a case report
The successful surgical treatment of medically refractory epilepsy is based on one of three different principles: (1) elimination of the epileptic focus, (2) interruption of the pathways of neural propagation, and (3) increasing the seizure threshold through cerebral lesions or electrical stimulation. Temporal lobe epilepsy, being the most common focal epilepsy, may ultimately require temporal ...
متن کاملAlternative surgical procedures to help drug-resistant epilepsy - a review.
The concepts of pathophysiology of epilepsy which underly the non-resective surgical treatment of epilepsy are reviewed. The available techniques, lesioning, disconnection and stimulation are described and reviewed critically. Stereotactic lesioning, popular in the 1950's has been largely abandoned but stereotactic radiosurgery emerges as a useful technique, especially in the treatment of mesia...
متن کاملStereotactic radiosurgery for treatment of epilepsy.
Stereotactic radiosurgery, known for efficacy in the noninvasive treatment of tumors or arteriovenous malformations that are otherwise difficult to access through traditional surgery, is an emerging technology in the treatment of focal epileptic lesions. Recent studies in the treatment of hypothalamic hamartoma and mesial temporal lobe epilepsy further suggest that seizures in these medically i...
متن کاملRobot Assisted Stereotactic Laser Ablation for a Radiosurgery Resistant Hypothalamic Hamartoma
Hypothalamic hamartomas (HH) are benign tumors that can cause significant morbidity in adults as a cause of epilepsy, particularly gelastic seizures. Open and endoscopic resections of HH offer good seizure control but have high rates of morbidity and are technically challenging. Stereotactic radiosurgery has been an alternative treatment; however, it results in comparably poor seizure control. ...
