چکیده
605 The earliest description of calcific aortic stenosis is attributed to the French physician Lazare Rivière who, in 1663, reported the necropsy findings from a patient with palpitations, progressive dyspnea, and loss of peripheral pulses. He noted left ventricular enlargement and identified large caruncle-like masses obstructing the left ventricular outflow to the aorta. Although other reports followed, the debate about the etiology of aortic valve calcification (AVC) did not begin until much later. Although calcification was originally attributed to endocarditis, in 1846, Hasse challenged this theory by suggesting that a degenerative process occurred within the valve itself to initiate ossification. In 1904, Möenckeburg authored the first detailed description of AVC in which he reported that this pathology likely occurred as a result of calcium deposition on valve cusps that had become sclerotic. He described 2 mechanisms to explain this phenomenon: degeneration within the layers of the valve leaflets nearest the sinuses of Valsalva that propagated toward the tips of the cusps or sclerotic changes of the aorta that extended to involve the valve cusps. Within the past 20 years, there has been a tremendous resurgence in interest in advancing these early hypotheses to identify the cellular and molecular mechanisms that initiate AVC owing to the high prevalence of this disease, its associated morbidity and mortality, and the emerging idea that therapies that target these processes could increase the durability of surgically implanted and transcatheter bioprosthetic valves.

The essential role of basic science in medical education: the perspective from psychology.
Probably ever since the Flexner report endowed basic scientists with a central role in the education of doctors, the same basic scientists have been engaged in active, but circular, debates about why basic science really is essential to medical education. The arguments are generally at the theological level and can easily be dismissed by critics as self-serving. There is little evidence in the ...
متن کاملThe articulation of integration of clinical and basic sciences in concept maps: differences between experienced and resident groups
To determine the content of integrated curricula, clinical concepts and the underlying basic science concepts need to be made explicit. Preconstructed concept maps are recommended for this purpose. They are mainly constructed by experts. However, concept maps constructed by residents are hypothesized to be less complex, to reveal more tacit basic science concepts and these basic science concept...
متن کاملBridging the gap between basic science and clinical practice: The role of organizations in addressing clinician barriers
BACKGROUND New National Institutes of Health policies call for expansion of practice-based research to improve the clinical research enterprise and facilitate dissemination of evidence-based medicine. OBJECTIVE This paper describes organizational strategies that influence clinicians' decisions to participate in clinical research. DESIGN We reviewed the literature and interviewed over 200 cl...
متن کاملPsychopathology as the basic science of psychiatry.
We argue that psychopathology, as the discipline that assesses and makes sense of abnormal human subjectivity, should be at the heart of psychiatry. It should be a basic educational prerequisite in the curriculum for mental health professionals and a key element of the shared intellectual identity of clinicians and researchers in this field.
متن کاملBridging the gap between basic science and clinical practice: a role for community clinicians
BACKGROUND Translating the extraordinary scientific and technological advances occurring in medical research laboratories into care for patients in communities throughout the country has been a major challenge. One contributing factor has been the relative absence of community practitioners from the US biomedical research enterprise. Identifying and addressing the barriers that prevent their pa...
