Bone peg arthrodesis of the first metatarsophalangeal joint.

چکیده

In I952 Duncan McKeever described a method of arthrodesing this joint, by means of a screw inserted across the excised joint, the head of the screw lying under the proximal phalanx, the screw itself occupying the whole of the medulla of the metatarsal shaft. He appeared to be wholly satisfied with the results, but patients who have had this type of fusion have, on occasion, complained of pain over the screw head on walking, with loss of the efficient functioning of the long flexor of the toe, an all-important factor in\ securing a satisfactory gait. In addition, the presence of the metallic foreign body in the metatarsal shaft has on one occasion provoked a bony reaction with periosteal new bone formation, and cavitation of the metatarsal, accompanied by severe pain. These complications have necessitated removal of the screw, a task which is not always simple and which subjects the patient to a second operative procedure. The method of fusion to be described obviates these complications.

First Metatarsophalangeal Joint Arthrodesis: A Retrospective Comparison of Crossed-screws, Locking and Non-Locking Plate Fixation with Lag Screw

Background:Locking plate fixation is increasingly used for first metatarsophalangeal joint (MTP-I) arthrodesis. Still there is few comparable clinical data regarding this procedure. Methods:We retrospectively evaluated 60 patients who received an arthrodesis of the MTP-I between January 2008 and June 2010. With 20 patients each we performed a locking plate fixation with lag screw, arthrodesis w...

A simple method of arthrodesis of the first metatarsophalangeal joint.

Eighty-seven feet have been reviewed after arthrodesis of the first metatarsophalangeal joint stabilised by chromic catgut. Radiological union was present in 90% of patients at the mean review time of six years. Twenty-four patients had significant metatarsalgia before operation and only two failed to improve afterwards. Excellent or good subjective results were reported in 94% of patients at r...

The management of the painful first metatarsophalangeal joint in the older patient. Arthrodesis or Keller's arthroplasty?

We report a prospective randomised trial comparing Keller's arthroplasty and arthrodesis of the first metatarsophalangeal joint for the management of symptomatic hallux valgus and hallux rigidus in the older patient. In 81 patients (110 feet), with a minimum of two years follow-up, both procedures gave a similar degree of patient satisfaction and symptom relief. The incidence of metatarsalgia w...

Evaluation of the Intermetatarsal Angle after the Arthrodesis of the First Metatarsophalangeal Joint for Treatment of the Hallux Valgus

OBJECTIVE To evaluate the correction of the intermetatarsal angle after arthrodesis of the metatarsophalangeal joint of the hallux. We believe that varus deformity of the first metatarsal can be corrected after arthrodesis of the first metatarsophalangeal joint, without the need for proximal osteotomy. METHODS Forty-three feet of patients who had undergone arthrodesis of the first metatarsoph...
