Binaural aids and sound localization: nothing is easy.

چکیده
C onventional wisdom holds that hearing-impaired individuals ought to be able to localize sounds better with two hearing aids than with one . But, in our featured article in this issue of JAAA, three antipodal colleagues : Dennis Byrne, William Noble, and Bernadette LePage show that this may be an oversimplification of a much more complex scenario . They begin by urging an important terminologic distinction between binaural hearing and the bilateral fitting of hearing aids, reminding us that bilateral fittings do not necessarily produce true binaural hearing, and that a unilateral fitting does not necessarily preclude true binaural hearing. In a group of 87, largely elderly, hearing aid users, Byrne et al measured aided sound localization ability in both the horizontal and vertical planes . Experienced hearing aid users who had been fitted either unilaterally or bilaterally were seated before an array of 20 loudspeakers and asked to identify, by number, the source speaker of a target burst of pulsed pink noise . Results showed that, while there was an overall advantage for persons with bilateral fittings as compared to unilateral fittings, the result was by no means uniform across users . Bilateral advantage was clearly related to degree of hearing loss . For persons with moderate and severe losses, there was a consistent bilateral advantage in localization, but for persons with only mild losses, the unilaterally fitted often did as well as, and sometimes even better than, the bilaterally fitted . Byrne et al conclude :

A method to induce swapped binaural hearing.
This paper describes the application of a small hearing aid that precisely fits into a subject's ear canal (complete-in-canal, or CIC). The bandwidth of the device is about 7 kHz. The system allows for selective manipulation of the different acoustic cues used for sound localization. The potential of the system is illustrated by robustly interchanging the input of the left and right ear, and co...
متن کاملEvaluation of Speech Intelligibility and Sound Localization Abilities with Hearing Aids Using Binaural Wireless Technology
Wireless synchronization of the digital signal processing (DSP) features between two hearing aids in a bilateral hearing aid fitting is a fairly new technology. This technology is expected to preserve the differences in time and intensity between the two ears by co-ordinating the bilateral DSP features such as multichannel compression, noise reduction, and adaptive directionality. The purpose o...

OBJECTIVE The objective of the study was to evaluate the effectiveness of binaural dichotic presentation using comb filters with complementary magnitude responses, based on fixed bandwidth and auditory critical bandwidth, in improving speech perception by persons with moderate bilateral sensorineural hearing loss, and to assess its effect on localization of the sound source. DESIGN AND STUDY ...
متن کاملSonic Localization Cues for Classrooms: A Structural Model Proposal
We investigate sonic cues for binaural sound localization within classrooms and present a structural model for the same. Two of the primary cues for localization, interaural time difference (ITD) and interaural level difference (ILD) created between the two ears by sounds from a particular point in space, are used. Although these cues do not lend any information about the elevation of a sound s...
متن کاملFrom The Department of Clinical Neuroscience Karolinska Institutet, Stockholm, Sweden BILATERAL HEARING AIDS FOR BILATERALLY HEARING-IMPAIRED PERSONS – ALWAYS THE BEST CHOICE?
The most logical solution if a person has a hearing impairment on both ears, seems to be bilateral hearing aid amplification in both ears. Numerous studies have shown the advantages of this fitting strategy. However, there were always some individuals not benefiting from bilateral hearing aids; until recently a neglected group of persons. The first part of the present work examines patterns of ...
