چکیده

Prostaglandin E1 (Alprostadil), a drug used for ductal patency in cyanotic congenital heart disease, comprises an essential part of the treatment of infants waiting for cardiac surgery in hospitals with no specialized medical staff or in situations of inadequate weight for surgery. Prostaglandin E1 (PgE1) can also be used in elevation of presurgical oxygenation status in more severe cases. The duration of infusion is often short (from 6 hours to 20 days). However, in some cases, when there is low birth weight, infection, or absence of a specialized hospital to transfer the patient to (all of these being common situation in developing countries), the infusion time can extend for weeks or months. Common side effects of prostaglandin therapy include apnea, fever, convulsions, rash, skin flushing, vasodilatation with hypotension, diarrhea, and gastric outlet obstruction. These effects are related to short-term use of PgE1, and the Pediatric Intensive Care Unit staff has the expertise to easily recognize and treat them. In rare situations of prolonged use of alprostadil, the most important side effects include soft tissue swelling and reversible cortical hyperostosis in the large bones. We describe the case of an infant presenting this complication secondary to prolonged use of PgE1, with typical and extensive radiological findings.

[Neonatal cortical hyperostosis. A side effect of prolonged prostaglandin E1 infusion].

Prostaglandin E1 (PGE1) infusion is usually administered for short periods to maintain patency of ductus arteriosus in infants with cyanotic heart disease. Prolonged therapy may be necessary while patients are awaiting surgical treatment. Several side effects occur at the onset of the treatment, most of them reversible once the treatment is discontinued. Cortical hyperostosis is a frequent comp...

Prolonged prostaglandin E1 therapy in a neonate with pulmonary atresia and ventricular septal defect and the development of antral foveolar hyperplasia and hypertrophic pyloric stenosis

Prostaglandin E1 (alprostadil) is widely used for maintaining the patency of ductus arteriosus in ductus-dependent congenital heart defects in neonates to improve oxygenation. Among more common side effects are fever, rash, apnoea, diarrhoea, jitteriness, and flushing. More severe side effects are brown fat necrosis, cortical hyperostosis, and gastric outlet obstruction, most commonly the resul...

Generalized periosteal reaction and tissue swelling secondary to prolonged prostaglandin E1 infusion and venous stasis: a case report.

Prostaglandin E1 (PGE1) is the drug of choice for providing ductal patency in cyanotic congenital heart disease (CCHD) for a short period of time until essential surgical management. Occasionally, prolonged use of PGE1 is required when the surgical procedure is delayed due to certain clinical conditions. Prolonged use of PGE1 may lead to bone and tissue changes such as pretibial and soft tissue...

Bilateral vocal fold paralysis and dysphagia secondary to diffuse idiopathic skeletal hyperostosis.

BACKGROUND Diffuse idiopathic skeletal hyperostosis (DISH) is an idiopathic spinal disease common in the elderly and characterized by flowing ossification and osteophyte formation along the spinal column. Cervical hyperostosis is capable of producing dysphagia, stridor, and airway obstruction; however, there are no extant reports of true paralysis of bilateral vocal folds in patients fulfilling...
