چکیده

Endoscopic retrograde cholangiopancreatography (ERCP) is essential for the diagnosis and treatment of pancreatobiliary diseases but is invasive and may induce various complications, such as pancreatitis, cholangitis, hemorrhage and perforation. For successful and safe ERCP procedures first, initial selective cannulation without complications is essential. However, even in experts, selective biliary access may be incomplete in 5-10% of patients despite the various endoscopic techniques available to facilitate selective biliary access. For overcoming difficult cannulation, various technical tips are available and tried. In this section, we reviewed about various precut methods and wire-guided techniques for facilitating selective biliary access.

Optimal Use of Wire-Assisted Techniques and Precut Sphincterotomy

Various endoscopic techniques have been developed to overcome the difficulties in biliary or pancreatic access during endoscopic retrograde cholangiopancreatography, according to the preference of the endoscopist or the aim of the procedures. In terms of endoscopic methods, guidewire-assisted cannulation is a commonly used and well-known initial cannulation technique, or an alternative in cases...

Burdick's Technique for Biliary Access Revisited

The precut sphincterotomy is used to facilitate selective biliary access in cases of difficult biliary cannulation. Needle-knife precut papillotomy is the standard of care but is associated with a high rate of complications such as pancreatitis, duodenal perforation, bleeding, etc. Sometimes during bowing of the sphincterotome/cannula and the use of guide wire to facilitate biliary cannulation,...

Can wire-guided cannulation reduce the risk of post-endoscopic retrograde cholangiopancreatography pancreatitis? A meta-analysis of randomized controlled trials.

BACKGROUND AND AIM The use of wire-guided cannulation (WGC) for prevention of post-endoscopic retrograde cholangiopancreatography (ERCP) pancreatitis (PEP) is controversial. The aim of the present study was to assess the overall effect of WGC for PEP compared with conventional contrast-assisted cannulation by carrying out a meta-analysis of all available randomized controlled trials (RCT). ME...

Impact of combined precut techniques on selective biliary cannulation.

INTRODUCTION simple biliary cannulation techniques obtain a success rate of 80-95%. Advanced precut techniques are not immediately successful in 10-40% of cases. Between extreme options such as a second attempt a few days later or an immediate attempt by the transparietal route, an alternative precut technique may be used as a rescue strategy for the initially failed procedure. The purpose of t...

Combined precut in difficult biliary cannulation.

AIM precut sphincterotomy refers to a variety of endoscopic techniques that are used in order to access the bile duct when conventional methods of cannulation have failed. There are not significant data (such as efficacy, safety) about the use of different techniques of precutting at the same session. We have described our experience with combined precut sphincterotomy (CPS) and we have compare...
