She was otherwise asymptomatic and afebrile. Clinical examination revealed a large ballotable right-sided abdominal mass. Ultrasound revealed an enlarged right kidney with loss of the normal renal architecture, which was replaced with multiple hypoechoic nodules. A number of renal calculi were observed with typical acoustic shadowing (figure 1). A contrastenhanced CT scan demonstrated the diagnostic ‘bear’s paw’ sign, multiple calculi, retroperitoneal lymphadenopathy and a small right psoas abscess (figure 2). A dimercaptosuccinic acid (DMSA) scan demonstrated a non-functioning kidney (figure 3). Urine cultures tested positive for Proteus mirabilis. The patient initially underwent a percutaneous nephrostomy and received antibiotics. Subsequently, a radical nephrectomy was performed. Extensive inflammatory adhesions to the inferior vena cava, liver and surrounding tissues were found. She recovered fully postoperatively and remains well.

Xanthogranulomatous pyelonephritis report of a case in a 5 year old boy
Xanthogranulomatous pyelonephritis is an unusual type of the kidney infection which is characterized by infiltration of mononuclear inflammatory cells and lipid laden macrophages ( foam cells). It is manifested by a firm, lobulated and yellowish kidney mass which is difficult to differentiate from renal cell carcinoma grossly. Xanthogranulomatous pyelonephritis (XGP) is a destructive inflammatory process which is frequently caused by recurrent urinary tract infections or chronic obstruction by kidney stones. We present a 56-year-old female with an extensive retroperitoneal urinoma and xanthogranulomatous pyelonephritis of the lower pole moiety in a kidney with a duplicated collecting system due to obstructive nephrol...

We present a case of a patient with xanthogranulomatous pyelonephritis (XGP) presenting with a large (18 × 12 cm) left-sided flank mass with worsening left flank pain. CT abdomen/pelvis demonstrated a left kidney with parenchyma replaced by multiple large hypodense collections containing fluid and gas, a left staghorn calculus, and a communication between the kidney and large flank collection. ...

Diagnosis is usually made very late due to changing manifestations and infrequent occurrence. متن کاملRenal Squamous Cell Carcinoma Presented With Bone Metastasis and Coexistence With Xanthogranulomatous Pyelonephritis: A Case Report
Squamous cell carcinoma (SCC) of the kidney is a rare entity. Coexistence of SCC with xanthogranulomatous pyelonephrits is exceedingly rare with only few reports in the literature. We report a case of a 45-year-old male patient with xanthogranulomatous pyelonephritis coexistence with renal SCC in one kidney, which proved radiologically and histopathologically. The patient presented to the medic...
