More than 70% of the patients with heart failure symptoms have underlying coronary disease, whereas the majority of patients with ischaemic cardiomyopathy have a previous myocardial infarction. 2 The prognosis of these patients is poor, particularly in men, and many of them need intermittent hospitalisation because of decompensated heart failure. In the near future, chronic heart failure will be encountered even more often, as our population ages. 2 The clinical characteristics of the patient, the presence of ischaemia, and severity of left ventricular dysfunction determine the prognosis in heart failure. Traditionally, ischaemic left ventricular dysfunction was considered an irreversible process. In patients with ischaemic cardiomyopathy, medical treatment was regarded as the only management option. However, more than two decades ago, Diamond and Rahimtoola recognised that left ventricular dysfunction in these patients is not always permanent and launched the theory of myocardial hibernation. 4 Hibernating myocardium is defined as chronic reversible contractile dysfunction secondary to coronary artery disease. The presence of hibernating myocardium has become an additional motive to perform coronary revascularisation. In fact, if a substantial amount of hibernating myocardium is present, coronary revascularisation may substantially improve regional and global contractile function in selected patients with ischaemic cardiomyopathy. This article will focus on the clinical importance and identification of myocardial viability in patients with chronic ischaemic left ventricular dysfunction.

Imaging techniques for the assessment of myocardial hibernation. Report of a Study Group of the European Society of Cardiology.

This report of an ESC Study Group reviews current knowledge on myocardial hibernation and relevant imaging techniques, and provides an algorithm for investigation and management when a patient presents with ischaemic left ventricular dysfunction. It covers the definitions of myocardial viability, stunning and hibernation, it reviews the morphological findings in hibernation and it describes rel...

Tracing ischemic memory by metabolic pathways: BMIPP and beyond

Myocardial ischemia (MI) resulting in infarction is an important cause of mortality and morbidity worldwide. Acute ischaemia rapidly impairs myocardial contractile function. Myocardial dysfunction persisting for several hours after transient non-lethal ischaemia, eventually resulting in full functional recovery is termed as myocardial stunning. Hibernation is now thought to be...

Myocardial Viability: What We Knew and What Is New

Some patients with chronic ischemic left ventricular dysfunction have shown significant improvements of contractility with favorable long-term prognosis after revascularization. Several imaging techniques are available for the assessment of viable myocardium, based on the detection of preserved perfusion, preserved glucose metabolism, intact cell membrane and mitochondria, and presence of contr...

Myocardial Viability by DE-MRI and Low-dose Dobutamine CMR

Introduction Dysfunctional myocardium that remains viable has the potential for contractile recovery after reperfusion. Dysfunctional but viable myocardium has been broadly divided into two closely linked pathophysiological states, myocardial hibernation and stunning. Stunned myocardium is the result of an ischemic insult leading to contractile dysfunction despite adequate reperfusion. Hibernat...

The histology of viable and hibernating myocardium in relation to imaging characteristics.

OBJECTIVES This study characterizes the histology of myocardium predicted to be hibernating using three different imaging techniques to explain the discordance among them. BACKGROUND Both radionuclide and functional imaging techniques were used to assess myocardial hibernation. The former have high sensitivity and the latter high specificity for predicting functional recovery. METHODS Ninet...
