چکیده

particularly difficult cases in whom standard treatment had failed. In three of them mexiletine alone controlled the arrhythmia, but only at the cost of intolerable adverse effects. Combined treatment with mexiletine and propranolol, which have different actions and different adverse effects, enables arrhythmia to be suppressed through co-operative antiarrhythmic action. Since adverse effects have been a major limitation to widespread use of mexiletine3 4combined therapy with propranolol may broaden its usefulness as an antiarrhythmic drug by two means-by decreasing its adverse effects and by more effective arrhythmia suppression through co-operative drug action.

Severe prekallikrein (Fletcher factor) deficiency due to a compound heterozygosis (383Trp stop codon and Cys529Tyr).

We investigated a family with prekallikrein deficiency, using both standard coagulation tests and molecular biology techniques. The propositus was found to be a compound heterozygote for a Trp383 stop codon and a Cys529Tyr point mutation. The former mutation was located in exon 11, the latter in exon 14. The propositus inherited the first defect from his father and the second from his mother. B...

Immunopathogenesis of Immune Glomerular Diseases

This paper reviews current concepts of glomerular immune injury of both inflammatory and noninflammatory types. In noninflammatory lesions induced by antibody alone or C5b-9, the glomerular epithelial cell appears to be the principal target of injury. Similar mechanisms are probably operative in human diseases such as minimal change nephrotic syndrome and membranous nephropathy. In inflammatory...

Inhibition of thrombin generation in human plasma by phospholipid transfer protein

BACKGROUND Plasma phospholipid transfer protein (PLTP) transfers lipids between donors and acceptors (e.g., from HDL to VLDL) and modulates lipoprotein composition, size, and levels. No study has reported an assessment of the effects of PLTP on blood clotting reactions, such as reflected in thrombin generation assays, or on the association of venous thrombosis (VTE) risk with PLTP activity. M...

Minimal change disease: A case report of an unusual relationship.

Kidney injury associated with lymphoproliferative disorders is rare, and the exact pathogenetic mechanisms behind it are still poorly understood. Glomerular involvement presenting as a nephrotic syndrome has been reported, usually secondary to membranoproliferative glomerulonephritis. We report a case of a 63-year-old male who presented with bilateral leg swelling due to nephrotic syndrome and ...

Minimal change nephropathy associated with anaplastic carcinoma of bronchus.

Nephrotic syndrome associated with extra-renal malignancy shows a characteristic pattern: carcinoma is most frequently associated with membranous glomerulonephritis and Hodgkin's disease with minimal change nephropathy. We report a patient with minimal change nephropathy in association with anaplastic carcinoma of the bronchus. Strong temporal evidence of a causal relationship is presented. The...
