Paracetamol is a commonly used NSAID (Non-steroidal anti-inflammatory agent) since its introduction by Von Mering in 1893. Its popularity is because of its long safety record and good efficacy in children and adults. Though it has been used for so many years, its mechanism of action is yet to be elucidated. NSAIDS inhibit prostaglandin synthesis by inhibiting COX-1(Cyclooxygenase-1) and COX-2(Cyclooxygenase 2) enzymes and produce analgesic, antipyretic and anti-inflammatory activity. Paracetamol like other NSAIDS produce analgesic and antipyretic effects, but is a poor anti-inflammatory agent which suggests a different mechanism of action. COX-3 a novel isoform was discovered to unravel the mystery surrounding paracetamol mechanism of action. Similarly, multiple mechanism of actions rooted in an effort to explain this differential pharmacological actions. This review is an effort to unravel this mystery by critically analyzing the available literature on paracetamol mechanism of action.
