چکیده

Islet transplantation can provide metabolic stability for patients with type 1 diabetes; however, more than one donor pancreas is usually required to achieve insulin independence. To evaluate possible mechanistic defects underlying impaired graft function, we studied five subjects at 3 months and four subjects at 12 months following intraportal islet transplantation who had received comparable islet equivalents per kilogram (12,601 1,732 vs. 14,384 2,379, respectively). Cpeptide responses, as measures of -cell function, were significantly impaired in both transplant groups when compared with healthy control subjects (P < 0.05) after intravenous glucose (0.3 g/kg), an orally consumed meal (600 kcal), and intravenous arginine (5 g), with the greatest impairment to intravenous glucose and a greater impairment seen in the 12-month compared with the 3-month transplant group. A glucose-potentiated arginine test, performed only in insulin-independent transplant subjects (n 5), demonstrated significant impairments in the glucose-potentiation slope (P < 0.05) and the maximal response to arginine (ARmax; P < 0.05), a measure of -cell secretory capacity. Because ARmax provides an estimate of the functional -cell mass, these results suggest that a low engrafted -cell mass may account for the functional defects observed after islet transplantation. Co-Transplantation of VEGF-Expressing Human Embryonic Stem Cell Derived Mesenchymal Stem Cells to Enhance Islet Revascularization in Diabetic Nude Mice

Background: Pancreatic islet transplantation has emerged as a promising treatment for type I diabetes. However, its efficacy is severely hampered due to poor islet engraftment and revascularization, which have been resulted to partially loss of transplanted islets. It has been shown that local delivery of vascular endothelial growth factor (VEGF) could accelerate transplanted islet revasculari...

Human islet amyloid polypeptide oligomers disrupt cell coupling, induce apoptosis, and impair insulin secretion in isolated human islets.

Insulin secretion from the 2,000-3,000 beta-cells in an islet is a highly synchronized activity with discharge of insulin in coordinate secretory bursts at approximately 4-min intervals. Insulin secretion progressively declines in type 2 diabetes and following islet transplantation. Both are characterized by the presence of islet amyloid derived from islet amyloid polypeptide (IAPP). In the pre...

Death and dysfunction of transplanted β-cells: lessons learned from type 2 diabetes?

β-Cell replacement by islet transplantation is a potential curative therapy for type 1 diabetes. Despite advancements in islet procurement and immune suppression that have increased islet transplant survival, graft function progressively declines, and many recipients return to insulin dependence within a few years posttransplant. The progressive loss of β-cell function in islet transplants seem...

استفاده از مهار کننده‌ فاکتور نسخه‌برداری NF - κB در جزایر پانکراس

Background: Pancreatic islet transplantation has been reported as an appropriate method for treatment of type I diabetes patients, however there are strong indications that cytokine and chemokines secreted from transplanted islets play an important role in islet graft rejection in different stage post-transplantation. The NF-kB signaling pathway is activated in response to the stress resulted f...

Outcome of human islet isolation and allotransplantation in 20 consecutive cases.

This report provides our initial experience on islet isolation and intrahepatic allotransplantation in 20 patients. In Group 1, 10 patients underwent combined liver-islet allotransplantation following upper-abdominal exenteration for cancer. One patient underwent pancreatic islet allograft after near total pancreatectomy for chronic pancreatitis. In Group 2, 3 Type I diabetic patients received ...
