چکیده

OBJECTIVE Conventional methods for managing upper-extremity (UE) spasticity are invasive, usually require readministration after a certain time period, and do not necessarily increase UE function. This study examined efficacy of combining two singularly efficacious modalities-UE bracing and electrical stimulation-with functional training to reduce UE spasticity and improve function. METHOD Two chronic stroke patients exhibiting UE spasticity were administered the Modified Ashworth Scale (MAS), the upper-extremity section of the Fugl-Meyer Impairment Scale (FM), the Box and Block Test (B&B), and the Arm Motor Ability Test (AMAT). They were then individually fitted for a brace and subsequently participated in treatment sessions occurring 2 days/wk for 5 wk, consisting of (1) 30-min clinical sessions, during which the UE was braced in a functional position while cyclic electrical stimulation was applied to the antagonist extensors of the tricep and forearm, and (2) 15-min, clinically based training sessions, occurring directly after the clinical session. RESULTS After intervention, participants exhibited 1-point reductions in MAS scores for the affected fingers, FM score increases, and increased ability to perform AMAT activities,. Three months later, both participants retained these changes. CONCLUSION Data point to a noninvasive, promising method of managing spasticity and rendering functional changes.

Neuromuscular electrical stimulation and dynamic bracing for the management of upper-extremity spasticity in children with cerebral palsy.

A prospective study was designed to determine whether the combined use of neuromuscular electrical stimulation (NMES) and dynamic bracing was more effective than use of either alone in reducing upper-extremity spasticity in children with spastic hemiplegic cerebral palsy. Twenty-four patients (12 males, 12 females; mean age 8y 7mo [SD 4y 2mo]; age range 3-18y) diagnosed with spastic hemiplegic ...

Botulinum Toxin in the Treatment of Upper and Lower Limb Spasticity Post Stroke

Botulinum toxin is significantly associated with decreased spasticity, increased range of motion and improved upper extremity function. Botulinum toxin in combination with electrical stimulation improves upper extremity function. Deinnervation of the subscapularis muscle using botulinum toxin may reduce shoulder pain and improve passive range of motion, more so than deinervation of the pectoral...

Effect of intensive functional electrical stimulation therapy on the upper limb motor recovery after stroke: Single case study of a chronic stroke patient

This case report describes a stroke patient who participated in an intensive functional electrical stimulation (FES) therapy that consisted of task-specific upper arm movements with a combination of pre-programmed electrical stimulation and manually assisted motion. The patient was a 22-year-old woman who had suffered a hemorrhagic stroke 2 years earlier. Motor function of the upper extremity w...

Impact of Extension Splint on Upper Extremity Motor Components and Function in Chronic Stroke Patients

Objectives: Spasticity and contracture in flexor muscles of the wrist may occur after stroke, especially in which early recovery did not appear. Splints are prescribed to reduce spasticity and to prevent contracture after stroke. Although there is a few research in this field. The aim of this study was to examine the impact of the Extension splint on function, spasticity, and range of motion of...

Botulinum Toxin Injections or Application of Splints: Impact on Spasticity, Range of Motion and Function of Upper Extremity in Chronic Stroke Patients

Spasticity or increase in muscle tone is one of the problems following stroke. Due to this increase in muscle tone, patients are confronted to problems in motor control and difficulties in activities of daily living and complications such as shortness and contracture. The aim of this study is to compare the effects of using splint or botulinum toxin injection on spasticity, range of motion and ...
