Transition from atrioventricular node reentry tachycardia to atrial fibrillation begins in the pulmonary veins.

چکیده
A37-year-old man with frequent episodes of paroxysmal atrial fibrillation refractory to antiarrhythmic therapy (and no other documented arrhythmias on multiple Holter examinations) underwent an attempt at radiofrequency ablation of the atrial fibrillation. Catheter positions are shown in the left anterior oblique view in Figure 1 and include 2 catheters in the left and right upper pulmonary veins. During mapping, the patient developed atrioventricular node reentry tachycardia (AVNRT) both spontaneously and with atrial extrastimuli; this AVNRT was later ablated. Before ablation, the patient’s rhythm spontaneously changed from AVNRT to atrial fibrillation (Figure 2). The catheter in the right upper pulmonary vein clearly recorded pulmonary venous potentials, which initiated atrial ectopic beats (as seen in the change of the activation sequence in the coronary sinus) and subsequent atrial fibrillation. Atrial fibrillation is a common occurrence in patients with supraventricular tachycardias, including AVNRT, and often starts during the supraventricular tachycardia.1 The mechanism by which atrial fibrillation begins during supraventricular tachycardias is unknown, although spontaneous atrial premature depolarizations have been cited as a trigger.2 Furthermore, the mechanism behind why atrial fibrillation recurs after successful ablation of supraventricular tachycardia is unknown.3 As seen in this patient, 1 site from which spontaneous ectopy can initiate

Arrhythmias in Patients With Pulmonary Hypertension; A Narrative Review of the Current Literature
Incidence and prognostic relevance of supraventricular arrhythmias were evaluated in nine studies. These studies investigated patients with different forms of pulmonary hypertension. Supraventricular arrhythmias were more often found in patients with group two pulmonary hypertension. Common findings in these patients were elevated right atrial pressure and diameters and reduced tricuspid annula...
متن کاملMechanisms of atrial fibrillation: is a cure at hand?
The mechanisms of atrial fibrillation relate to the presence of random reentry involving multiple interatrial circuits. Triggers for development of atrial fibrillation include rapidly discharging atrial foci (mainly from pulmonary veins) or degeneration of atrial flutter or atrial tachycardia into fibrillation. Therapy for control of atrial fibrillation includes drugs, atrial pacing for those w...
متن کاملArrhythmogenicity of the Coronary Sinus
The coronary sinus (CS) is the cardiac venous system that begins at its ostium in the right atrium and ends at the origin of the great cardiac vein.1,2,3 The major tributaries of the CS include the great cardiac vein (anterior cardiac vein), the left obtuse marginal vein, the posterior (or inferior) left ventricular vein, the middle cardiac vein, and the right coronary vein. In addition, atrial...
متن کاملModulation of extracellular atrioventricular node field potential pattern and ventricular rhythm by morphine in experimental atrial fibrillation in isolated rabbit heart
Introduction: Endorphins are produced by cardiomyocytes, and exert different effects on the heart. The aim of the present study is to assess morphine effects on extracellular atrioventricular (AV) node field potential pattern and ventricular rhythm of isolated rabbit heart during experimental atrial fibrillation (AF). Methods: Effects of different concentrations of morphine (10, 20, 50 and 1...
متن کاملProtective effects of simvastatin on atrioventricular node during simulated experimental atrial fibrillation in vitro
Introduction: Recent evidence has indicated that statins can reduce the incidence of both supraventricular and ventricular arrhythmias with various mechanisms. The primary goal of the present study was to determine direct protective role of simvastatin in modifying concealed conduction and the zone of concealment in a simulated model of atrial fibrillation (AF) in an isolated atrioventricula...
