Teaching neuroimages: MRI in advanced neuromyelitis optica.

چکیده
A 51-year-old woman was treated with multiple medications for relapsing-remitting multiple sclerosis (MS) over 20 years, including interferons and glatiramer, but continued to have recurrent attacks of optic neuritis and transverse myelitis, leading to bilateral blindness and triplegia. Even with MS-type lesions onMRI, neuromyelitis optica (NMO) was ultimately suspected and confirmed by detection of aquaporin-4 autoantibodies. MRI showed nonenhancing T2 linear hyperintensity around the ventricular system in areas known to highly express aquaporin-4 (figure). Profound spinal cord atrophy was evident, consistent with severe, chronic NMO. This case illustrates the lack of effectiveness and potential detriment when MS-directed immunomodulatory medications are aimed at NMO.

Teaching neuroimages: neuromyelitis optica misdiagnosed as spinal cord tumor.
A 17-year-old girl presented with acute-onset cervical pain, followed by left arm weakness and gait disturbances. Spinal cord astrocytoma was diagnosed by MRI performed at an outpatient facility (figure, A and B). The patient was admitted to the neurosurgery department to undergo spinal cord biopsy. A second neurologic evaluation indicated neuromyelitis optica (NMO) as the most likely diagnosis...

A 48-year-old man presented with ascending sensory deficits over 12 hours, followed by urinary retention. He had areflexia, mild lower extremity weakness, sensory ataxia, and a T2 sensory level. Smooth pursuit was impaired, but cranial nerves were otherwise normal. Diagnostic evaluation demonstrated CSF cytoalbuminologic dissociation and demyelinating polyneuropathy fulfilling the electrodiagno...
متن کاملTeaching NeuroImages: Leber hereditary optic neuropathy masquerading as neuromyelitis optica.
A 22-year-old woman presented with photopsia, sensory loss, and paresthesia over both lower extremities, visual scotomas, and painless binocular vision loss over 5 months. MRI showed T2 hyperintensities near the optic chiasm (figure 1, B and C), floor of the fourth ventricle and the colliculi (figure 2, A–F), and central gray matter of the spinal cord (figure 1A). CSF analysis was notable for e...
متن کاملImaging Surrogates of Disease Activity in Neuromyelitis Optica Allow Distinction from Multiple Sclerosis
Inflammatory demyelinating lesions of the central nervous system are a common feature of both neuromyelitis optica and multiple sclerosis. Despite this similarity, it is evident clinically that the accumulation of disability in patients with neuromyelitis optica is relapse related and that a progressive phase is very uncommon. This poses the question whether there is any pathological evidence o...

Neuromyelitis optica (also known as Devic's disease or Devic's syndrome) is an uncommon disorder in pediatric age group, and is characterized by acute or subacute optic neuritis and transverse myelitis. Here we report an 11- year- old female child with relapsing Neuromyelitis optica (NMO) confirmed by positive NMO- IgG antibody and had clinical recovery with high dose methyl prednisolone t...
