HRCT Profile in Diagnosing Active Pulmonary Tuberculosis

چکیده
Background -The purpose of this study is to show the role of high-resolution CT scan (HRCT) in diagnosing active pulmonary tuberculosis. tree-in bud appearance in HRCT is more sensitive than chest X-ray in detection of early endobronchial spread, 97% of patients with active pulmonary TB had evidence of bronchogenic spread of the disease on HRCT. Materials & Methods-One hundred clinically suspected of having active pulmonary TB with chest-Xray appearances suggestive of the disease, underwent HRCT examination. Results-HRCT results showed that 76 patients had active pulmonary TB and further clinical work-up indicated that the final diagnosis of the disease was confirmed in 52 patients. According to binomial test the combination of the two main HRCT appearances, the centrilobular nodule and "tree-in-bud" signs confirms TB diagnosis accurately. The sensitivity of HRCT in diagnosing active pulmonary TB in this study was 96% and the negative predictive value was calculated as 93%. Conclusion-We conclude that HRCT is a powerful and reliable diagnostic tool in TB diagnosis, which means that it can be used even before mycobacteriologic results are available.

High resolution computed tomographic findings in pulmonary tuberculosis.
BACKGROUND Although chest radiographs usually provide adequate information for the diagnosis of active pulmonary tuberculosis, minimal exudative tuberculosis can be overlooked on standard chest radiographs. The aim of the present study was to assess the findings of active pulmonary tuberculosis on high resolution computed tomographic (HRCT) scans, and to evaluate their possible use in determini...
متن کاملFiberoptic bronchoscopy for the rapid diagnosis of smear-negative pulmonary tuberculosis
BACKGROUND This study was aimed to investigate the diagnostic value of fiberoptic bronchoscopy (FOB) with chest high-resolution computed tomography (HRCT) for the rapid diagnosis of active pulmonary tuberculosis (PTB) in patients suspected of PTB but found to have a negative sputum acid-fast bacilli (AFB) smear. METHODS We evaluated the diagnostic accuracy of results from FOB and HRCT in 126 ...
متن کاملDiagnosing fibrotic lung disease: when is high-resolution computed tomography sufficient to make a diagnosis of idiopathic pulmonary fibrosis?
Idiopathic pulmonary fibrosis (IPF), a progressive and fatal diffuse parenchymal lung disease, is defined pathologically by the pattern of usual interstitial pneumonia (UIP). Unfortunately, a surgical lung biopsy cannot be performed in all patients due to comorbidities that may significantly increase the morbidity and mortality of the procedure. High-resolution computed tomography (HRCT) has be...
متن کاملHRCT and whole-blood interferon-gamma assay for the rapid diagnosis of smear-negative pulmonary tuberculosis.
BACKGROUND Early diagnosis of active pulmonary tuberculosis (PTB) is critical for TB control, and difficult in patients with smear-negative sputum. OBJECTIVE We wanted to evaluate the usefulness of clinical findings, high-resolution computed tomography (HRCT), interferon-gamma-releasing assay (IGRA) and polymerase chain reaction (PCR) of sputum in the diagnosis of smear-negative PTB. METHOD...
متن کاملThe Correlation of Brody High Resolution Computed Tomography Scoring System with Clinical Status and Pulmonary Function Test in Patients with Cystic Fibrosis
Background: To reduce the mortality and morbidity rates of cystic fibrosis (CF) patients, and to have an effective clinical management, it is important to monitor the progression of the disease. The aim of this study was to evaluate the progression of lung disease in CF patients by means of assessing the correlation of the CT scoring system with clinical status and pulmonary function test at th...
