چکیده

Fourteen patients with classic tic douloureux were treated with neurovascular decorapression of the trigeminal nerve at its zone of entry into the pons. In addition, selective rhizotomy of the portio major was performed in each patient. This combined procedure gave excellent relief of pain with no recurrences in a 23-month postoperative period. Technical observations and postoperative complications in 3 patients are discussed. Routine selective partial rhizotomy of the sensory root of the trigeminal nerve may be a useful adjunct to neurovascular decompression for tic douloureux. Identification of Meckel cave during percutaneous glycerol rhizotomy for tic douloureux.

A new percutaneous treatment of tic douloureux has been developed . Transovale injection of glycerol into the retrogasserian trigeminal fibers in Meckel cave results in complete pain relief in 77%-89% of patients [1-3]. The major advantages of percutaneous retrogasserian glycerol rhizotomy include simplicity, radiograph ic confirmation of accurate need le placement, and a much reduced incidence...

Vascular Decompression in Trigeminal Neuralgia

Trigeminal neuralgia (TN) is a sudden, severe, brief, stabbing and recurrent pain within one or more branches of the trigeminal nerve. Although multiple mechanism involving peripheral pathologies at root (compression or traction), dysfunctions of brain stem, basal ganglion and cortical pain modulatory mechanisms could have role, neurovascular conflict (NCV) is most accepted theory. Patients wit...

Electromagnetic Induction Aberration: The Possible Mechanism of Tic Douloureux

A theory based on the principles of electromagnetic induction aberration is presented as the possible mechanism of classic trigeminal neuralgia, tic douloureux. The anatomy of the dorsal root entry zone of the trigeminal nerve at the pons in the proximity of the superior cerebellar artery presents a scenario conducive to the phenomenon of electromagnetic induction. When the action potentials tr...

Gas CT cisternography of trigeminal neuralgia caused by AVM of the cerebellopontine angle.

Recently, percutaneous thermocoagulation [1-3] or retrogasserian glycerol rhizotomy [4, 5] has been used in the treatment of tic douloureux. In a few unsuccessful cases, lesions such as multiple sclerosis, tumors of the cerebellopontine angle, arteriovenous malformation (AVM), or aneurysm of the vertebrobasilar artery system have been reported as the causative factors [5] . Among these, AVM is ...

Glossopharyngeal neuralgia caused by a complex neurovascular conflict: Case report and review of the literature

BACKGROUND Glossopharyngeal neuralgia (GN) is a rare condition characterized by severe, paroxysmal episodes of pain mainly localized to the external ear canal, pharynx, and tongue, usually caused by a neurovascular conflict between postero-inferior cerebellar artery (PICA) and IX cranial nerve. Sometimes there is also a compression of X c.n. CASE DESCRIPTION We present a case of a 71-year-old...
