[Lung volume reduction surgery for pulmonary emphysema].

چکیده
Surgical treatment for pulmonary emphysema has received renewed attention, because many studies have demonstrated improvements in dyspnea and pulmonary function. After lung volume reduction surgery (LVRS) for carefully selected advanced pulmonary emphysema, pulmonary functional impairments, such as expiratory flow limitation, increase in lung volume, decrease in lung elastic recoil, impaired gas exchange are improved. Improvements of dynamic hyperinflation, respiratory muscle function and exercise tolerance were also demonstrated after LVRS for pulmonary emphysema. Finally these functional effects may improve symptom and prognosis of pulmonary emphysema with better quality of life. Although the basic mechanism responsible for the improvements are not well known yet, physiological mechanisms which brought improvement after surgical treatment for pulmonary emphysema should be further investigated.

Lung volume reduction surgery and lung transplantation in chronic obstructive pulmonary disease
Medical treatment of emphysema does not alter the natural progression of the disease. Surgical techniques are an attractive conceptual approach to treat hyperinflation in these patients. Lung volume reduction surgery and lung transplantation are appropriate therapeutic options for a selected population with emphysema. We will review the available evidence to support these approaches.
متن کاملBronchoscopic lung volume reduction for emphysema: where next?
E mphysema is an irreversible disease and although patients derive significant benefit from inhaled therapies [1] and pulmonary rehabilitation [2], many remain significantly disabled. The National Emphysema Treatment Trial of lung volume reduction clearly showed that in appropriately selected patients it is possible to improve lung function, exercise capacity, quality of life and survival [3, 4...
متن کاملSurgical therapies for chronic obstructive pulmonary disease.
Surgical procedures for treating emphysema were first developed nearly 100 years ago. Despite a wide range of surgical procedures performed over the years, only three appear to have true clinical benefit: bullectomy, lung volume reduction surgery (LVRS), and lung transplantation. Lung volume reduction surgery has been reintroduced in the past decade and is currently under active research. A rec...
متن کاملFunctional and morphological heterogeneity of emphysema and its implication for selection of patients for lung volume reduction surgery.
Lung volume reduction surgery (LVRS) in patients with advanced pulmonary emphysema aims to alleviate symptoms and enhance quality of life by improving respiratory mechanics. The theoretical concepts of the operation predict the greatest functional benefit in patients with marked hyperinflation, and with airflow obstruction due to loss of elastic recoil. Consistent observations in several centre...

STUDY OBJECTIVES This study reports histopathologic findings in a group of emphysema patients who underwent thoracoscopic lung volume reduction surgery (75) or sternotomy (five) with the purpose to induce functional improvement and relief of dyspnea. Immediate outcome and complications were correlated to histologic patterns. DESIGN Histopathologic material obtained in lung volume reduction su...
