چکیده

16 Objective: This study develops normalized values for sensation in the feet of healthy subjects. Knowledge relative to normal sensory perception and subsequent loss will enable clinicians to differentiate between normal and abnormal findings, especially in patients with or at risk for diabetes. Methods: Subjects were over age 18 and without medical complications that might compromise sensory perception. Subjects were divided into three age groups: 18-34, 35-64, and 65 & older. With the subject's eyes closed, Semmes Weinstein monofilaments were applied to 10 sites per foot and progressively lightened each time the subject correctly identified the monofilament at 10/10 sites, until the subject's sensory threshold was reached. The threshold was defined as the lightest monofilament sensed at all 10 sites. Additionally, the researcher ranked the condition of the subject's feet on a 1-3 scale, and the subject completed a questionnaire regarding their average weekly frequency of sandal use.001) different than the other groups. The older two groups did not significantly differ. There was a significant (-.445, n=60, p<0..0001) negative correlation between the frequency of sandal use and foot sensitivity. Conclusions: Foot sensitivity decreases with age, but decline initiates earlier than anticipated. Loss of peripheral nerve fibers and integumentary sensory organs are implicated as the cause for sensory loss. Additionally, foot sensation decreased with increased sandal use.

Improved sensitivity in patients with peripheral neuropathy: effects of monochromatic infrared photo energy.

The medical records of 1,047 patients (mean age, 73 years) with established peripheral neuropathy were examined to determine whether treatment with monochromatic infrared photo energy was associated with increased foot sensitivity to the 5.07 Semmes-Weinstein monofilament. The peripheral neuropathy in 790 of these patients (75%) was due to diabetes mellitus. Before treatment with monochromatic ...

Symptomatic reversal of peripheral neuropathy in diabetic patients

Objective: There is no therapy which reverses peripheral neuropathy (PN) in most diabetic patients. We hypothesized that a FDA cleared medical device, which emits monochromatic near infrared (890 nm) photo energy (MIRE) would improve neural function lost during diabetes. Research Design arid Methods: 49 consecutive diabetic subjects (Type 1. n=25: Type II, n=24). with established neuropathy rec...

Symptomatic reversal of peripheral neuropathy in patients with diabetes.

Forty-nine consecutive subjects with established diabetic peripheral neuropathy were treated with monochromatic near-infrared photo energy (MIRE) to determine if there was an improvement of sensation. Loss of protective sensation characterized by Semmes-Weinstein monofilament values of 4.56 and above was present in 100% of subjects (range, 4.56 to 6.45), and 42 subjects (86%) had Semmes-Weinste...

Reversal of diabetic peripheral neuropathy and new wound incidence: the role of MIRE.

OBJECTIVE To determine if improved foot sensitivity to the Semmes-Weinstein 10-g (5.07) monofilament, originally impaired because of diabetic peripheral neuropathy, might be associated with a reduced incidence of new diabetic foot wounds. DESIGN Retrospective cohort study using a health status questionnaire. SUBJECTS Sixty-eight individuals over age 64 with diabetes, diabetic peripheral neu...

The threshold for protective sensation that prevents neuropathic ulceration on the plantar aspect of the foot: a study of leprosy patients in a rural community in India.

The protective sensation threshold is an important concept in the prevention of plantar ulceration in leprosy patients. Previous studies have suggested that skin with sensory nerve damage on the plantar aspect of the foot which can still detect the 5.07 Semmes-Weinstein monofilament (approximately 10 g) is highly unlikely to develop ulceration. While the threshold is thought to be less than the...
