An Uncommon Combination of Fractures around the Elbow: Capitellum Fracture Associated with Radial Head Fracture

چکیده
INTRODUCTION The coexistence of fractures of capitulum humeri and radial head in the elbow joint is a rare entity. CASE REPORT A 30-year-old woman was admitted to our hospital after having sustained a fall on her outstretched right arm. She complained of pain, swelling, and restriction of motion of the right elbow joint. Anteroposterior and lateral radiographs revealed displaced fractures in the capitellum and the radial head. The patient was operated. Fractures were exposed through a lateral (Kocher) incision. Fracture was reduced, and fixation was performed with 2 AO compression screw. Two fragments of radial head fracture were also reduced and fixed with AO compression screw. Clinical and radiographic outcomes were excellent at the end of a 5-year follow-up. CONCLUSION These fractures occurred following a fall on the palm with outstretched arm. The success of the treatment depends greatly on surgical approach and suitable technique which requires comprehensive knowledge of detailed anatomy and biomechanics of the elbow.

Terrible triad of the elbow: a case report of a new variant.
The usual terrible triad of the elbow consists of posterior dislocation of the elbow, radial head fracture and coronoid fracture. We describe a new variant of the terrible triad of the elbow consisting of fracture of the capitellum involving the full length of the trochlea and posterolateral dislocation of the elbow associated with coronoid fracture (type 1 Regan-Morrey). A 25-year-old girl was...
متن کاملEvaluation of Radiocapitellar Arthritis in Patients with a Second Radiograph at Least 2 Years after Nonoperative Treatment of an Isolated Radial Head Fracture
Background: To study if patients that have a second radiograph 2 or more years after nonoperative treatment of an isolated radial head fracture have radiocapitellar osteoarthritis (RC OA). Methods: We used the database of 3 academic hospitals in one health system from 1988 to 2013 to find patients with isolated radial head fractures (no associated ligament injury or fracture) that had a second ...
متن کاملFractures of the capitellum humeri and their associated injuries
Objective Fractures of the capitellum are rare injuries but are often more complex and of a greater extent than assumed from conventional radiographs. Classification is usually based on their extension in relation to the trochlea the trochlea and on the number of fragments. Information on associated injuries is limited and only reported in small case series. The aim of this retrospective study ...
متن کاملSynchronous fractures of the trochlea and the radial neck without elbow dislocation.
Fractures of the articular surface of the distal end of the humerus (i.e., of the capitellum and the trochlea) are rather uncommon injuries. Fractures of the capitellum account for 1% of all elbow fractures and about 6% of all distal humeral fractures. Fractures of the trochlea are usually encountered as part of more complex intra-articular fractures of the distal end of the humerus, disrupting...
متن کاملMonteggia fractures in pediatric and adult populations.
A Monteggia fracture is a fracture of the proximal ulna coupled with a radial head dislocation. These fractures are an uncommon class of forearm fractures. Numerous classification systems have been developed to characterize these fractures, with the Bado classification being the most common. Elbow radiographs are the primary diagnostic modality, demonstrating dislocation when a line drawn exten...
