"Intermittent Hæmorrhage" from Malarial Influence

on the 22nd of February last, Surgeon-Major J. H. Porter of Netley read a paper on the above subject, and illustrated it by a case in which amputation of the thigh, through the condyles, was followed by recurring secondary haemorrhage, each flow being preceded by hot and cold stages. Tlie man had been invalided from India in April 1875, " when he was very weak, anaemic and emaciated," and subsequently the knee-joint became disorganised. Sulphate of quinine was administered, and the patient " made an excellent recovery. Dr. Lauder B run ton considered the paper of great value, as illustrating the two points bearing upon the seat of the poison of ague in the body, and the action of that poison." He considered that it supported Lusana's theory that the poison of ague having passed through the liver, is excreted in tlie bile, and taken up by the portal blood. " When however the poison enters the systemic circulation a fit of ague is produced." In the case cited, " two circumstances hud operated to divert the portal blood into the systemic circulation, namely, loss of blood and shook." Then followed contraction of the cutaneous and intestinal vessels, and in consequence a free out-pouring of biood from the divided soft parts and bones. This occurred upon five different days. Dr. Brunton further added :?" The phenomena of the attack of ague are due to contraction of the cutaneous vessels; and similar symptoms can be produced by experimental irritation of the vaso-motor centre in the medulla.
