Use of Fluoroscopy for Interlaminar Lumbar Epidural Injection: Is Confirmation with Epidurogram Necessary?
نویسنده
چکیده
We compared the loss of resistance (LOR) technique and fluoroscopy with the LOR technique, fluoroscopy, and epidurography for interlaminar epidural injection. A total of 371 patients with symptomatic lumbosacral radiculopathy were enrolled; 17 had a history of allergy to iodinated contrast medium or seafood. Each patient was given an oral steroid before and after the procedure. A Tuohy needle was inserted into the epidural space using the LOR technique and fluoroscopy for guidance. After confirming that the needle was in the epidural space, 3 cc of nonionic contrast medium was injected and an epidurogram was performed. No patient with a history of allergy had an adverse reaction to the nonionic contrast medium. This study suggests that the routine use of an epidurogram, even in patients with a history of allergy to iodinated contrast, after preparation with oral steroids, is justified.

Inadvertent Intrafacet Injection during Lumbar Interlaminar Epidural Steroid Injection: A Comparison of CT Fluoroscopic and Conventional Fluoroscopic Guidance.
BACKGROUND AND PURPOSE Inadvertent intrafacet injection can occur during interlaminar epidural steroid injection, resulting in a false-positive loss of resistance and nontarget injection of medication. The purpose of this investigation was to compare the observed rates of this phenomenon during lumbar interlaminar epidural steroid injection performed by using conventional fluoroscopic and CT fl...
متن کاملA complication during caudal steroid injection.
BACKGROUND AND OBJECTIVE Epidural injections in the lumbar spine are provided by caudal, lumbar interlaminar or transforaminal routes. Caudal epidural steroid injections are often used for low back pain. Fluoroscopic guidance has been frequently cited as a requirement for this procedure. In this case report, we demonstrate the importance of fluoroscopic guidance during caudal epidural injection...
متن کاملIncorrect needle position during lumbar epidural steroid administration: inaccuracy of loss of air pressure resistance and requirement of fluoroscopy and epidurography during needle insertion.
Loss of air pressure resistance leads to a high rate (25.7%) of inaccurate needle-tip placement in the posterior soft tissues of the back during lumbar epidural steroid administration employing a 20-gauge Tuohy needle. Imaging and epidurogram are essential for confident identification of the lumbar epidural space to enable accurate location of steroid administration. Studies assessing efficacy ...
متن کاملAccuracy of Live Fluoroscopy to Detect Intravascular Injection During Lumbar Transforaminal Epidural Injections
BACKGROUND Complications following lumbar transforaminal epidural injection are frequently related to inadvertent vascular injection of corticosteroids. Several methods have been proposed to reduce the risk of vascular injection. The generally accepted technique during epidural steroid injection is intermittent fluoroscopy. In fact, this technique may miss vascular uptake due to rapid washout. ...
