Small Diameter Hysteroscopy: Is it a Real Opportunity?

چکیده
The hysteroscopy is the gold standard for treatment of uterine intracavitary diseases. It is effective and safe with low rate of recurrence and complications. The features of intrauterine lesions are factors influencing the instrument choice. The larger are generally treated by resectoscope but this technique is associated with potential serious complications mainly due to the need of cervical dilatation. To overcome this limitation, technological improvements have led to the introduction of small-diameter hysteroscopes not exceeding 5mm in diameter equipped with bipolar electrodes that work in saline solution. The vaginoscopic approach avoids the use of speculum and tenaculum, and the small caliber of the instruments avoids cervical dilatation; therefore small-diameter hysteroscopes are more suitable. However, the major challenge in the use of small diameter hysteroscopy is the extraction of tissue fragments through a narrow cervical canal. Therefore, hysteroscopic morcellation is introduced. It provides a fast, effective and safe alternative to bipolar resectoscopy in the hysteroscopic treatment of endometrial polyps, but future trials are required for assess cost-effectiveness and cost-benefit ratio. To date, there are no guidelines defining the limits of the size, location and number beyond which small diameter hysteroscopy should be preferred to the resectoscopy. However, the indications for smalldiameter operative hysteroscopy are expanding and not yet completely defined; in experienced hands a small-diameter hysteroscopy is a safe and effective approach for benign endometrial disease up to 4cm, especially in patients with unfavourable cervical canal at risk of cervical injury.

"See-and-Treat" Hysteroscopy in the Management of Endometrial Polyps.
Endometrial polyps (EPs) are a common cause of abnormal uterine bleeding (AUB) in perimenopausal and postmenopausal women and are typically suggested by a screening transvaginal ultrasound. In addition, the increasing use of pelvic imaging often discloses asymptomatic EPs. In the past, saline infusion sonography (SIS) has been advocated in order to triage patients to undergo a blind curettage o...

Uterine adenomyosis is a common gynecologic disorder in women of reproductive age, characterized by the presence of ectopic endometrial glands and stroma within the myometrium. Dysmenorrhea, abnormal uterine bleeding, chronic pelvic pain, and deep dyspareunia are common symptoms of this pathological condition. However, adenomyosis is often an incidental finding in specimens obtained from hyster...

Intrauterine pathologies have been shown to be present in 25% of infertile patients.Structural abnormalities of the uterine endometrial cavity may affect the reproductive outcome adversely, by interfering with the implantation or causing spontaneous abortion. Intrauterine abnormalities may visualize using a variety of techniques. Including hysterosalpingography (HSG), transvaginal sonography (T...
متن کاملI-23: The Role of Hysteroscopy in Patients with Repeated IVF Failure
The success of reproduction, although gradually increasing over the years, many couples also had been left frustrated following repeated failed attempts. Implantation of the embryo, can only take place in a receptive uterus and inadequate uterine receptivity is responsible for approximately two-thirds of implantation failures. Endometrial receptivity can be reduced by morphological and/or molec...
متن کاملModelling Intravasation of Liquid Distension Media in Surgical Simulators
We simulate the intravasation of liquid distention media into the systemic circulation as it occurs during hysteroscopy and transurethral resection of the prostate. A linear network flow model is extended with a correction for non-newtonian blood behaviour in small vessels and an appropriate handling of vessel compliance. We then integrate a fast lookup scheme in order to allow for real-time si...
