Early breast cancer in elderly women: surgery or primary endocrine therapy?
نویسندگان
چکیده
Background Breast cancer is the most common type of cancer in women. Up to 30% of all breast cancers are reported to occur in the over 70 year age group and these percentages may increase in the near future as the mean age of the general population grows up. The standard treatment for early breast cancer in women of all ages was surgery until the late 1970s, when primary endocrine therapy was first described as an alternative to standard therapy for older women (Bradbeer 1983). Primary endocrine therapy means the use of Tamoxifen, without surgery, radiotherapy or chemotherapy. The incidence of significant co-morbidity is greater in the elderly, making general anaesthesia more hazardous.

Primary Endocrine Therapy in Older Women with Breast Cancer
Purpose of Review Breast cancer incidence increases with age. In recent years, primary endocrine therapy has been increasingly used as a treatment option for frail elderly women with breast cancer, although surgery is still the guideline-recommended treatment. In this review, we discuss the evidence for primary endocrine therapy versus surgical treatment in older women with early breast cancer....

BACKGROUND Older women are at the greatest risk of breast cancer development and a considerable number present with comorbidities. Although the majority of breast cancers in this age group express oestrogen receptor (ER), which makes endocrine therapy (primary or adjuvant) feasible, given the huge size of the elderly population, there remains a significant number of patients, in absolute term, ...

Primary endocrine therapy (PET) is often included as a treatment option in elderly women with operable breast cancer. Elderly women tend to have pre-existing comorbidities and are often reluctant to undergo surgery. The benefit of surgery needs to be weighed against a relatively higher potential for operative morbidity and mortality, and a limited life expectancy. But while PET can provide rela...
متن کاملManaging early-stage breast cancer in your older patients.
As the aging population in the United States continues to grow, the incidence of diseases of the elderly, such as breast cancer, are increasing. Many more elderly women are expected to be diagnosed with new breast cancers, most of them in an early stage. Appropriate treatment of these women is important, as they have poorer outcomes when undertreated. In this review, we will discuss the biology...

BACKGROUND Tamoxifen as sole therapy (primary tamoxifen therapy) was investigated in the 1970s and 1980s as an alternative to surgery for older patients with breast cancer. While the majority of primary breast tumors responded to tamoxifen, long-term local disease control was poor. The use of primary tamoxifen therapy is therefore restricted to frail, elderly, and infirm patients who cannot tol...
