چکیده
It has been our sincerest honor to have guest-edited this focus issue on “Disk Degeneration and Pain” for the Global Spine Journal. Of particular importance is that this issue commemorates the “first” focus issue of our newly developed journal, which we hope will be one of many more to come. In a recent publication by Vos et al,1 published by The Lancet, low back pain was identified as “the leading” debilitating condition worldwide, representing a tremendous socioeconomic and health care burden. Although not always synonymous with each other, disk degeneration is regarded as one of the determinants related to the development of low back pain. As such, disk degeneration and low back pain have gained the attention of the medical, scientific, and public interest for many years. Nevertheless, these conditions are complex and multi-factorial, and their development and management continue to challenge spine specialists and researchers alike. Therefore, in an effort to expound on matters, we have tailored a unique focus issue composed of multi-disciplinary international experts that have come together in an effort to raise awareness of the current cuttingedge issues surrounding disk degeneration and pain, ranging from imaging and pathomechanisms to novel treatments. Adhering to review-style papers, this focus issue addresses the following:

Molecular Therapy for Disk Degeneration and Pain
The nucleus pulposus of the intervertebral disk contains high amounts of the proteoglycan aggrecan, which confers the disk with a remarkable ability to resist compression. Other molecules such as collagens and noncollagenous proteins in the extracellular matrix are also essential for function. During disk degeneration, aggrecan and other molecules are lost due to proteolysis. This can result in...
متن کاملMechanisms of Intervertebral Disk Degeneration/Injury and Pain: A Review
Degeneration of the intervertebral disk and its treatments are currently intensely investigated topics. Back pain is a condition whose chronic and debilitating nature combined with its prevalence make it a major health issue of substantial socioeconomic importance. Although researchers, and even sometimes clinicians, focus on the degenerated disk as the problem, to most patients, pain is the fa...
متن کاملDisk Degeneration and Low Back Pain: Are They Fat-Related Conditions?
Low back pain (LBP) is the world's most debilitating condition. Disk degeneration has been regarded as a strong determinant associated with LBP. Overweight and obesity are public health concerns that affect every population worldwide and whose prevalence continues to rise. Studies have indicated strong associations between overweight/obesity and disk degeneration as well as with LBP. This broad...
متن کاملInflammatory Mediators in Intervertebral Disk Degeneration and Discogenic Pain
Although degeneration of the intervertebral disk has historically been described as a misbalance between anabolic and catabolic factors, the role of inflammatory mediators has long been neglected. However, past research clearly indicates that inflammatory mediators such as interleukin (IL)-1β, IL-6, IL-8 and tumor necrosis factor-α are expressed at higher levels in "diseased" intervertebral dis...
متن کاملBMI, Abdominal Obesity and Physical Activity in Adult Candidates for Lumbar Disk Degeneration Surgery
Background and Aim: Lumbar disc degeneration is one of the most frequent causes of pain in the lower back, and lack of physical activity, overweight and obesity have been regarded as the most significant factors in its occurrence. The present study was conducted with the aim of evaluation of body mass index, abdominal obesity and physical activity level in patients registered for elective lumba...
