Migration of a percutaneous endoscopic gastrostomy tube into the transverse colon: a forgotten cause of refractory diarrhea.

چکیده
cutaneous endoscopic gastrostomy (PEG) tube inserted for palliation of dysphagia due to a large, epiphrenic esophageal diverticulum. After PEG feeding for 1 month, he complained of persistent, disabling diarrhea. Potential infective or organic causeswere excluded after extensive negative investigations and feeding regimen adjustment. An esophagogastroduodenoscopy (EGD) incidentally performed for investigation of anemia did not reveal the PEG button anywhere inside the stomach. Because of these peculiar findings, a contrast-enhancedcomputed tomography (CT) scanwas performed. Surprisingly, the PEG button was found inside the transverse colon (●" Fig. 1). Awater-soluble contrast study showed the PEG button had migrated into the midtransverse colon with antegrade contrast filling down to the sigmoid colon and retrograde fillingup to the cecum (●" Fig. 2). There was no abnormal communication between the colon and the stomach to indicate the presence of a gastrocolic fistula. Because of the absence of sepsis or peritonitis, the patient was managed conservatively and the PEG tube was removed. The associated colocutaneous fistula finally closed uneventfully 1 week later. Migration of the PEG tube into the colon with or without an associated gastrocolic fistula is an exceedingly rare complication of this relatively safe endoscopic procedure [1–4]. Most patients present subacutely with disabling diarrhea (50%) or faecal leakage (39%), several months after initial insertion or on tube replacement; a small minority (11%) is asymptomatic with the diagnosis established incidentally [4]. The underlying cause is mostly related to inadvertent puncture of the transverse colon and the stomach during initial insertion [5]. Subsequent excessive tension on the tube can result in complete migration via the iatrogenic gastrocolic fistula into the colon. Post-surgical adhesion, high-riding transverse colon, and stomach over-distension during EGD are known predisposing risk factors [4]. When such a complication is suspected, contrast study via the PEG tube is imperative in establishing the diagnosis. More than half of patients do not have a residual gastrocolic fistula on presentation [4]. Emergency laparotomy as part of the management of these patients is indicated only when peritonitis ensues, as otherwise, spontaneous closure of the colocutaneous and gastrocolic fistulas typically occurs after PEG tube removal. Inadvertent Percutaneous Endoscopic Gastrostomy Tube Placement through the Transverse Colon to the Stomach Causing Intractable Diarrhea: A Case Report
Background. Among patients with chronic disease, percutaneous endoscopic gastrostomy (PEG) tubes are a common mechanism to deliver enteral feedings to patients unable to feed by mouth. While several cases in the literature describe difficulties with and complications of the initial placement of the PEG, few studies have documented the effects of a delayed diagnosis of a misplaced tube. Methods....
متن کاملTwo Cases of Gastrocolocutaneous Fistula with a Long Asymptomatic Period after Percutaneous Endoscopic Gastrostomy
Gastrocolocutaneous fistula is a rare complication of the percutaneous endoscopic gastrostomy (PEG) procedure. Typical symptoms usually occur in the first few months. We recently encountered 2 patients with 8- and 33-month asymptomatic periods. A 74-year-old man presented with watery diarrhea for 1 month. He had undergone PEG 9 months earlier. During workup, an upper endoscopy and abdominal CT ...

An 84-year-old woman with vascular dementia and percutaneous endoscopic gastrostomy (PEG) placement 2 months prior presented with a fecal odor from the PEG tube and diarrhea at each feeding. No feces output from the PEG tube or peristomal leakage was noted. Computed tomography of the abdomen showed penetration of the PEG tube through the transverse colon to the stomach, without surrounding infl...
متن کاملSurgical approach to a delayed presentation of gastrocolocutaneous fistula following percutaneous endoscopic gastrostomy
Introduction: Gastrocolocutaneous fistula is a rare complication of percutaneous endoscopic gastrostomy tube placement that can present early at the time of insertion or late due to misplacement of gastrostomy tube through colon. It is suspected if there is profuse diarrhea after each tube feed, feculent drainage through or around the gastrostomy tube. Case Report: We report a rare case of gast...
متن کاملIntermittent diarrhea as a delayed presentation of percutaneous endoscopic gastrostomy (PEG)-associated fistula.
A 60-year-old patient with severe dysphagia, weight loss, and recurrent aspiration pneumonia required a percutaneous endoscopic gastrostomy (PEG) for long-term feeding. After 24 uneventful days, she developed an enigmatic recurring but intermittent diarrhea. On day 62, staff noted a feculent odor from her gastrostoma, along with undigested formula in her stools. This prompted her hospitalizatio...
