defining the femoral head and the acetabulum. The differentiation of the primitive chondroblasts leads to the formation of the femur, which is in the shape of a club. The anlagen of the ilium, ischium, and pubis develops into a disc-shaped mass. At the age of 11 weeks, the femoral head is fully formed with a spherical configuration, a short neck, and a primitive greater trochanter. 8 With continuous development of the labrum, the femoral head becomes deeply seated in the acetabulum and is held by the surface tension of the synovial fluid. The postnatal acetabular cartilage complex is a unit that is triradiate medially and cup-shaped laterally, interposing between the ilium, ischium, and pubis. This complex is composed of the epiphyseal growth plate cartilage adjacent to these bones, the articular cartilage around the acetabular cavity and, for the most part, hyaline cartilage. The interstitial growth within the triradiate part of the cartilage complex causes the hip socket to expand during growth, and the concavity of the acetabulum develops in response to the presence of the spherical femoral head. 9,10 At puberty, the depth of the acetabulum is further increased by the development of 3 secondary centres of ossification in the hyaline cartilage surrounding the acetabular cartilage. These centres are homologous with other epiphysis in the skeleton. As the acetabulum enlarges, a balance between the growth of the acetabular and triradiate cartilage and that of the adjacent bone induces normal depth of acetabulum. 9 In DDH, this balance may be at risk. In these patients, the degree of acetabular malformation at birth ranges from mild to severe. 10 A ridge in the ischial and Normal development of the hip joint requires the formation of a balanced and harmonious relationship between the growth of the acetabular and triradiate cartilages and the femoral head. 1,2 Developmental dysplasia of the hip (DDH) is a specific condition, in which the harmonious relationship is shattered. The most important concept in the management of DDH is the reduction of the femoral head into the true acetabulum by closed or open reduction to restore this relationship. 3 It is generally assumed that following a stable reduction, various components of the abnormal joint in infancy will develop satisfactorily into a normal joint. 4–6 Accordingly, every effort is made to reduce the dislocation as soon as possible for the development of a normal hip joint. Because of the maximum growth …

Reconstruction of the Acetabulum in Developmental Dysplasia of the Hip in Total Hip Replacement

Developmental dysplasia of the hip (DDH) or congenital hip dysplasia (CDH) is the most prevalent developmental childhood hip disorder. It includes a wide spectrum of hip abnormalities ranging from dysplasia to subluxation and complete dislocation of the hip joint. The natural history of neglected DDH in adults is highly variable. The mean age of onset of symptoms is 34.5 years for dysplastic DD...

Acetabular dysplasia after treatment for developmental dysplasia of the hip.

Untreated acetabular dysplasia following treatment for developmental dysplasia of the hip (DDH) leads to early degenerative joint disease. Clinicians must accurately and reliably recognise dysplasia in order to intervene appropriately with secondary acetabular or femoral procedures. This study sought early predictors of residual dysplasia in order to establish empirically-based indications for ...

Background and Objective: Arthrosis of the hip joint is one of the most common complications of patients and many efforts have been made to find its etiology. One of the subjects important for researchers is morphologic parameters of the hip joint and its relationship with osteoarthritis of this joint. Since these parameters themselves are affected by physical parameters of the body and the rac...

Background: Acetabular dysplasia is a well-known cause of early osteoarthritis of hip which may appear at any time (perinatal, breast-feeding and childhood). The aim of this study was to evaluate the clinical and radiographic outcomes of children with de-velopmental dysplasia of hip (DDH) after undergoing open reduction, capsulorrhaphy and Pemberton osteotomy procedures. Methods: This study ...

Improvement in acetabular index after reduction of hips with developmental dysplasia.

After late reduction of the hip in children with developmental dysplasia the acetabular contour slowly improves and it is difficult to know if and when acetabular reconstruction is required. We studied the radiographs of 19 patients with unilateral dislocated or subluxated hips which had been reduced between the ages of one and two years. Preoperatively, all the affected hips showed acetabular ...

Total hip replacement with a superolateral bone graft for osteoarthritis secondary to dysplasia: a long-term follow-up.

We evaluated the long-term results of 116 total hip replacements with a superolateral shelfplasty in 102 patients with osteoarthritis secondary to developmental dysplasia of the hip. After a mean follow-up of 19.5 years (11.5 to 26.0), 14 acetabular components (12%) had been revised. The cumulative survival at 20 years was 78%, with revision for loosening of the acetabular component as the end-...
