Endoscopic resection of a large pedunculated duodenal polyp using a grasping type scissors forceps.

چکیده
tally discovered during screening esopha− gogastroduodenoscopy in a 72−year−old woman. The polyp, located in the second portion of the duodenum, had a long stalk and a large head. Its removal was consid− ered to be technically demanding be− cause the head of the polyp occluded the lumen of the duodenum, making it diffi− cult to maneuver the snare over the polyp. The stalk part of the polyp, how− ever, was fully visible, and we tried endo− scopic resection using a newly developed grasping type scissors forceps (GSF) (XDP2618DT; Fujinon) (l" Fig. 1 and l" 2) [1], after obtaining written informed con− sent from the patient. A two channel endoscope was used. First, we applied three long clips (HX−600± 090L; Olympus) to the base of the stalk close to the duodenal wall for the preven− tion of polypectomy−associated bleeding. Second, the upper part of the stalk was held with the grasping forceps to avoid losing the polyp in the distal duodenum. Third, the polyp stalk was resected about 5 mm above the clip by GSF using auto cut mode 120W (l" Fig. 3). No hemor− rhage, perforation, or other complication occurred. Histological diagnosis was a Peutz−Jeghers type polyp. Large pedunculated gastrointestinal tract polyps are often difficult to remove by en− doscopic polypectomy with the standard snare technique [2± 4]. Our approach was to perform endoscopic resection with a GSF [1]. The GSF has a thin serrated cutting edge to facilitate grasping the tis− sue. The outer side of the forceps is insu− lated so that electrosurgical current ener− gy is concentrated at the blade to avoid burning the surrounding tissue. By using the GSF, it was safe and easy to resect the stalk of the polyp. We believe this tech− nique has the potential to become the method of choice for the removal of ped− unculated gastrointestinal tract polyps when other procedures involving snaring of the polyp head are not feasible.

Endoscopic resection of a giant pedunculated polyp using scissor-type forceps.
An 83-year-old man presented with a feeling of abdominal fullness and underwent colonoscopy. Two pedunculated polyps were identified in the sigmoid colon: the one on the oral side was about 20mm, while that on the anal side was a giant lesion nearly as large as the diameter of the bowel lumen (●" Fig.1a). It was judged to be difficult to perform polypectomy by snaring the latter polyp. Both pol...
متن کاملA novel strategy for complete duodenal endoscopic submucosal dissection involving prophylactic defect closure with over-the-scope clips.
Duodenal endoscopic submucosal dissection (ESD) remains controversial because of anatomical specificities, technical difficulties, and an unacceptably high rate of complications including delayed perforations and bleeding [1–3]. Accordingly, it is essential to establish a strategy to overcome these problems. A newly developed over-the-scope clip (OTSC; Ovesco Endoscopy, Tübingen, Germany) is no...

A 58-year-old man was referred for resection of a pedunculated polyp, which had a head measuring 3.5×2.8cm and a stalk 1.5cm in diameter, over the sigmoid colon. We initially applied a detachable loop over the stalk, but no total cyanotic change of the polyp head was observed. We applied a second loop, but it became entrapped in the head of the polyp without tightening (●" Fig.1). We attempted ...
متن کاملEndoscopic Resection of a Pedunculated Brunner's Gland Hamartoma of the Duodenum
A 68-year-old Japanese woman presented with a solitary pedunculated polyp in the duodenum. Endoscopic ultrasonography showed multiple cystic structures in the polyp. The polyp was successfully resected by endoscopic snare polypectomy and pathologically diagnosed as Brunner's gland hamartoma. Because hamartomatous components were not identified in the stalk of the polyp, we speculate that the st...

A case of pedunculated Brunner's gland hamartoma (BGH) of the duodenum causing upper gastrointestinal (GI) hemorrhage is reported. The patient was a 47-year-old man who visited our hospital for further evaluation of tarry stools and shortness of breath. Endoscopic examination of the upper digestive tract revealed a large peduncular polyp with bleeding, about 30 mm in diameter, arising from the ...
