Metaplasia in the ileal reservoir

چکیده
firmed these findings in a small group of patients. It appears that destruction of the glandular basement membrane and the immediately surrounding sheath of supporting cells prevents orderly regeneration. When applying a strict definition of atrophy, as proposed by Genta (8), it is highly unlikely that cure of H. pylon infection will result in restitutlo ad integrum i.e., glandular parenchyma regeneration and functional recovery. Few studies have given indications that intestinal metaplasia may reverse. Genta et al. (9) have shown in a small but detailed study that intestinal metaplasia regresses. In conclusion, the mechanisms leading to mutation of the genes in epithelial cells may be triggered very early in the H. pylon gastritis-gastric cancer sequence. Atrophic gastritis and intestinal metaplasia may result from these processes. Moreover, these changes would seem to be the point of no return.

Restorative proctocolectomy with ileal reservoir: pathological and histochemical study of mucosal biopsy specimens.
Mucosal biopsy specimens from the ileal reservoirs of 92 patients who had undergone restorative proctocolectomy (12 with familial adenomatous polyposis, 78 with ulcerative colitis, and two with functional bowel disease) were studied. Chronic inflammation was found in almost all, as was villous atrophy of varying severity. Other changes included pyloric metaplasia and mucosal prolapse. Acute inf...
متن کاملHistopathological evaluation and risk factors related to the development of pouchitis in patients with ileal pouches for ulcerative colitis
OBJECTIVE Many changes in mucosal morphology are observed following ileal pouch construction, including colonic metaplasia and dysplasia. Additionally, one rare but potential complication is the development of adenocarcinoma of the reservoir. The aim of this study was to evaluate the most frequently observed histopathological changes in ileal pouches and to correlate these changes with potentia...
متن کاملConfocal laser endomicroscopy for the detection of mucosal changes in ileal pouch after restorative proctocolectomy.
BACKGROUND Pouchitis and dysplasia may affect the reservoir after restorative proctocolectomy. AIMS To assess the suitability of confocal laser endomicroscopy for the in vivo diagnosis of mucosal changes in ileal pouch for ulcerative colitis and familial adenomatous polyposis. METHODS Standard endoscopy and endomicroscopy were performed in 18 pouches. Confocal images were scored for the pre...
متن کاملIntestinal permeability in the ileal pouch.
BACKGROUND Villous atrophy, mucin changes ('colonic metaplasia'), and chronic inflammation occur to varying degrees in all patients with ileal pouchanal anastomosis whereas acute inflammation (pouchitis) affects a subgroup of patients with prior ulcerative colitis. AIM To measure epithelial barrier function looking for possible functional adaptation in ileal 'pouch' mucosa. PATIENTS Patient...
متن کاملSquamous Cell Carcinoma of an Ileal Neobladder
Bladder reconstruction using bowel segments, especially the ileum, has become a realistic option for urinary diversion. There is only one prior case of squamous cell carcinoma of the ileal neobladder that has been reported in the clinical literature. Here we report a patient with a spectrum of squamous cell lesions, including squamous cell carcinoma, sarcomatoid carcinoma, squamous papilloma an...
